How did doctors make a living? Making a Medical Living explores the neglected socio-economic history of medical practice, beginning with the first voluntary hospital in 1720 and ending with national health insurance in 1911. It looks at public appointments in hospitals and dispensaries, office under public welfare systems, and at private practice. In this innovative study, Anne Digby makes use of new sources of information, looks at ordinary rather than élite doctors, and analyses provincial rather than metropolitan practice. From the mid-eighteenth century medicine became more commercialised; doctors travelled to see ordinary patients, developed specialisms, and were entrepreneurial in expanding institutional forms of health care. This entrepreneurial activity helped shape English medicine into a distinctive pattern of general and specialist practice, and of public and private health care.

For centuries, scholars have argued that envy is the source of much aggressive behavior as well as the root cause of much unhappiness, but it is only recently that there have been attempts to examine the emotion from an empirical perspective. This book is the first of its kind to offer a comprehensive summary of current theoretical and empirical work on envy provided by scholars from a range of disciplines. The first section of the book focuses on the rich theological, philosophical, and evolutionary foundations of scholarly thinking on envy. The second section covers the social psychological work on envy and includes chapters on social comparison processes, definitional challenges, the link between envy and schadenfreude, intergroup envy, and fear of envy. The third section covers research on envy from organizational psychology, experimental economics, marketing, neuroscience, and anthropology. The records of manorial courts have been used increasingly as the principal source for the reconstruction of rural and small town society in Medieval England. They offer a unique source with which to investigate peasant demography, family patterns, the village community and economy, the characteristics and instruments of customary law and the ways in which that law was perceived and exploited by landlords and tenants. The essays in this collection provide novel approaches to all of these themes and are written by many of the historians who have pioneered the use of this source category in the last two decades.In two introductory chapters, the editors review the historiography of manorial court rolls and account for their origins as a distinctive record of customary law within the broad context of medieval European society. It was during this period that for the first time, co-operation between phoneticians and teachers culminated in the publication of works that were instrumental in establishing the 'applied linguistic' approach to language teaching in the twentieth century. For the first time, too, the new science of psychology influenced a scientific theory of second language acquisition.The Reform Movement attracted support across Europe, spurring the development of new professional associations and journals. In turn, the publication in these journals of reports of innovative practice contributed to a greater sense of autonomy and professionalism among modern language teachers, who had hitherto tended to live under the shadow of classical language teaching. The practical innovations and theoretical suggestions for the foreign language teaching, although rooted in the nineteenth century, still have relevance today.

The new Conservative government, under Margaret Thatcher, was determined to strengthen British defences against the perceived Soviet threat and advocated a strong response to the Soviet intervention. East-West relations further deteriorated following the imposition of martial law in Poland in December 1981. The dilemma facing the British government was how to express strong disapproval of Soviet actions while still attempting to maintain a constructive bilateral relationship, and at the same time keep British policy in line with the Western Alliance. The death of President Brezhnev in November 1982, after 18 years in office, brought uncertainty but also new opportunities for relations with the Soviets. This book will be of much interest to students of British politics and foreign policy, Russian history, US foreign policy, Central Asian politics, and IR in general.

Questions of discipline and order arise wherever formal education is practised, and are particularly acute for those training to teach or in their first school posts. For many years now writing on these topics has tended to depict teaching as the deployment of ‘skills’ and ‘techniques’ and competent teachers as those who successfully ‘manage’ their classes. This approach is criticised by Richard Smith as manipulative and destructive of the kind of pupil-teacher relationship conducive to any but the most trivial sorts of learning.Thus the philosophical issues which the book explores are shown throughout to have their roots in problems associated with established thinking and practice, and the author’s ideas have considerable practical relevance. He argues for a thorough reappraisal of the nature and basis of the teacher’s authority and demonstrates the importance of a proper understanding of the function of punishment. He suggests that many of the problems of discipline that teachers meet may actually stem from inappropriate ways of treating pupils, and shows that solutions to these problems must be compatible with the degree of initiative and personal responsibility that it is the business of education to foster. Schools have changed in many ways, largely for the better, since the first edition of this book appeared: the young people in them are generally treated with far more respect than was the case a quarter of a century ago. The voices of a more repressive tradition however still make themselves heard from time to time. It is therefore important continually to re-state the principles on which civilised relationships between pupils and teachers need to be based.

State Corporatism and Proto-Industry focuses on an industrial countryside in south-west Germany, where a dense worsted industry dominated the rural economy from 1580 to 1800. This is an example of 'proto-industry', the dense, export-oriented rural manufacturing which arose throughout Europe before factory industrialization. But although the Württemberg worsted industry possessed all the features of a classic proto-industry, closer scrutiny throws doubt on basic assumptions about European proto-industrialization. In this book, Sheilagh Ogilvie shows that proto-industries did not break down traditional society. Instead, corporate institutions such as guilds, merchant companies, village communities and manorial systems retained enormous power. This was a result of 'state corporatism': the expanding early modern state granted privileges to favoured groups in return for fiscal and regulatory co-operation. As Ogilvie shows, these corporate privileges profoundly constrained both individual decisions and economic development.

The traditional demographic regime of ancient Greece and Rome is almost entirely unknown; but our best chance for understanding its characteristics is provided by the three hundred census returns that survive on papyri from Roman Egypt. These returns, which date from the first three centuries AD, list the members of ordinary households living in the Nile valley: not only family members, but lodgers and slaves. The Demography of Roman Egypt has a complete and accurate catalogue of all demographically relevant information contained in the returns. On the basis of this catalogue, the authors use modern demographic methods and models to reconstruct the patterns of mortality, marriage, fertility and migration that are likely to have prevailed in Roman Egypt. They recreate a more or less typical Mediterranean population as it survived and prospered nearly two millennia ago.

The data are analysed within the framework of a structural model of mortality change comprising the proximate determinants of exposure to, and resistance against, infectious agents on the the part of populations. Within this framework a model is established describing the specific demographic and epidemiological characteristics of early modern metropolitan centres. The evidence indicates that mortality in London was much higher than in other settlements in England for most of the period, but declined steeply in the later eighteenth century. This apparently reflected changes in exposure to infections.

From the time of Malthus, the insufficient supply of food resources has been considered the main constraint of population growth and the main factor in the high mortality prevailing in pre-industrial times. In this essay, the mechanisms of biological, social and cultural nature linking subsistence, mortality and population and determining its short and long term cycles are discussed. The author's analysis examines the existing evidence from the century of the Great Plague to the industrial revolution, interpreting the scanty quantitative information concerning caloric budgets and food supply, prices and wages, changes in body height and epidemiological history, demographic behaviours of the rich and of the poor. The emerging picture sheds doubts on the existence of a long term interrelation between subsistence of nutritional levels and mortality, showing that the level of the latter was determined more by the epidemiological cycles than by the nutritional level of the population.

This book is the first full-length analysis of the London working population and the effects of the industrial revolution in London to appear for over sixty years. Prior to the mid nineteenth century London may not have experienced the direct effects of the industrial revolution to any great extent, but the indirect effects were felt strongly. L. D. Schwarz disagrees with the view that 'the industrial revolution was a storm that passed over London and broke elsewhere', and seeks to judge the effect of industrialisation on what was the country's largest manufacturing city. Its size and role as national capital meant that London was in certain important respects unique, but it was nonetheless susceptible to many of the wider economic transformations that occurred during the period 1700-1850, and Dr Schwarz offers a detailed analysis of the changes to the economy and social structure of London that these wrought.
