Green Healthcare Institutions : Health, Environment, and Economics, Workshop Summary is based on the ninth workshop in a series of workshops sponsored by the Roundtable on Environmental Health Sciences, Research, and Medicine since the roundtable began meeting in 1998. Environmental health decision making can be a complex undertaking, as there is the need to navigate and find balance among three core elements: science, policy, and the needs of the American public. Policy makers often grapple with how to make appropriate decisions when the research is uncertain. The challenge for the policy maker is to make the right decision with the best available data in a transparent process.The Environmental Health Sciences Decision Making workshop, the first in a series, was convened to inform the Roundtable on Environmental Health Sciences, Research, and Medicine on emerging issues in risk management, "weight of evidence," and ethics that influence environmental health decision making.The workshop, summarized in this volume, included an overview of the principles underlying decision making, the role of evidence and challenges for vulnerable populations, and ethical issues of conflict of interest, scientific integrity, and transparency. The workshop engaged science interest groups, industry, government, and the academic sector.

Houston is struggling with many of the environmental problems that most of the nation''s major metropolitan areas are struggling with - transportation, water and air pollution, flooding, and major demographic changes. Therefore, Houston provided an excellent site for a regional meeting on the relationship between environment and health. The purpose of this workshop in Houston was to bring all the stakeholders together - the private and public sector, along with representatives of the diverse communities in Houston - to discuss the impact of the natural, built, and social environments on human health. Rebuilding the Unity of Health and the Environment summarizes the presentations and discussions of this workshop. The lessons one may draw from this meeting''s presentations and discussions apply to other regions that are undergoing similar changes and that must also contend, as does Houston, with the legacies of insufficient planning, environmentally deficient planning, or sometimes, no planning at all.

The National Research Council''s Disasters Roundtable and the Institute of Medicine''s Roundtable on Environmental Health Sciences, Research, and Medicine were established as mechanisms for bringing various stakeholders together to discuss timely issues in a neutral setting. The goal was not to resolve these issues, but to create an environment conducive to scientific debate. The members of the respective Roundtables comprise representatives from academia, industry, nongovernmental agencies, and government, whose perspectives range widely and represent the diverse viewpoints of researchers, federal officials, and public interest. This report is the summary of a workshop was convened by the two Roundtables as a contribution to the debate on the health risks of disasters and the related need to build capacity to deal with them. The meeting was strengthened by integrating perspectives from these two fields, so that the agenda represented information from both communities and provided an opportunity to look at some of the most pressing research and preparedness needs for health risks of disasters.

The Rountable''s fifth national workshop entitled From Source Water to Drinking Water: Ongoing and Emerging Challenges for Public Health continued the theme established by previous Roundtable workshops, looking at rebuilding the unity of health and the environment. This workshop summary captures the discussions and presentations by the speakers and participants, who identified the areas in which additional research was needed, the processes by which changes could occur, and the gaps in our knowledge.

This report is the summary of the fourth workshop of The Roundtable on Environmental Health Sciences, Research, and Medicine. Environmental Indicators: Bridging the Chasm Between Public Health and the Environment, continues the overarching themes of previous workshops on rebuilding the unity of health and the environment. The purpose of the workshop was to bring people together from many fields, including federal, state, local, and private partners in environmental health, to examine potential leading indicators of environmental health, to discuss the proposed nationalhealth tracking effort, to look into monitoring systems of other nations, and to foster a dialogue on the steps for establishing a nationwide environmental health monitoring system. This workshop brought together anumber of experts who presented, discussed, and debated the issues surrounding the implementation of a monitoring system.

The Institute of Medicine''s Roundtable on Environmental Health Sciences, Research, and Medicine sponsored a workshop to understand the biological mechanism of normal labor and delivery, and how environmental influences, as broadly defined, can interact with the processes of normal pregnancy to result in preterm birth. This report is a summary of the main themes presented by the speakers and participants.

the Institute of Medicine Roundtable on Environmental Health Science, Research, and Medicine held a regional workshop in Pittsburgh, Pennsylvania, on March 13, 2003. This workshop was a continued outgrowth from the Roundtable''s first workshop when its members realized that the challenges facing those in the field of environmental health could not be addressed without a new definition of environmental health—one that incorporates the natural, built, and social environment. The Roundtable realized that the industrial legacy is not unique to Pittsburgh. Other cities around the world have seen their industries disappear, and it is only a matter of time before some of the Pittsburghs of today, such as Wuhan, China, (a sister city) will need to address similar problems. One goal for this IOM Environmental Health Roundtable Workshop is to extract lessons from Pittsburgh''s experience in addressing the post-industrial challenge, distilling lessons that might be useful elsewhere.

The purpose of this regional workshop in the Southeast was to broaden the environmental health perspective from its typical focus on environmental toxicology to a view that included the impact of the natural, built, and social environments on human health. Early in the planning, Roundtable members realized that the process of engaging speakers and developing an agenda for the workshop would be nearly as instructive as the workshop itself. In their efforts to encourage a wide scope of participation, Roundtable members sought input from individuals from a broad range of diverse fields-urban planners, transportation engineers, landscape architects, developers, clergy, local elected officials, heads of industry, and others. This workshop summary captures the discussions that occurred during the two-day meeting. During this workshop, four main themes were explored: (1) environmental and individual health are intrinsically intertwined; (2) traditional methods of ensuring environmental health protection, such as regulations, should be balanced by more cooperative approaches to problem solving; (3) environmental health efforts should be holistic and interdisciplinary; and (4) technological advances, along with coordinated action across educational, business, social, and political spheres, offer great hope for protecting environmental health. This workshop report is an informational document that provides a summary of the regional meeting.

As the field of environmental health continues to evolve, scientists and others must focus on gaining a better understanding of the links between human health and various environmental factors, and on creating new paradigms and partnerships needed to address these complex environmental health challenges facing society. Global Environmental Health in the 21st Century: From Governmental Regulations to Corporate Social Responsibility: Workshop Summary discusses the role of industry in environmental health, examines programs designed to improve the overall state of environmental health, and explores how governmental and corporate entities can collaborate to manage this industry.

Stakeholders in both the public and private sectors are looking for viable solutions as the complexity of societal problems and risks associated with management and varying regulatory standards continue to increase. Global Environmental Health in the 21st Century draws critical links and provides insight into the current shape of global environmental health. The book recommends expanding environmental management systems (EMS) to encompass a more extensive global network. It also provides a complete assessment of the benefits and costs resulting from implementation of various environmental management systems.

Public health officials have the traditional responsibilities of protecting the food supply, safeguarding against communicable disease, and ensuring safe and healthful conditions for the population. Beyond this, public health today is challenged in a way that it has never been before. Starting with the 9/11 terrorist attacks, public health officers have had to spend significant amounts of time addressing the threat of terrorism to human health. Hurricane Katrina was an unprecedented disaster for the United States. During the first weeks, the enormity of the event and the sheer response needs for public health became apparent. The tragic loss of human life overshadowed the ongoing social and economic disruption in a region that was already economically depressed. Hurricane Katrina reemphasized to the public and to policy makers the importance of addressing long-term needs after a disaster.On October 20, 2005, the Institute of Medicine''s Roundtable on Environmental Health Sciences, Research, and Medicine held a workshop which convened members of the scientific community to highlight the status of the recovery effort, consider the ongoing challenges in the midst of a disaster, and facilitate scientific dialogue about the impacts of Hurricane Katrina on people''s health. Environmental Public Health Impacts of Disasters: Hurricane Katrina is the summary of this workshop. This report will inform the public health, first responder, and scientific communities on how the affected community can be helped in both the midterm and the near future. In addition, the report can provide guidance on how to use the information gathered about environmental health during a disaster to prepare for future events.

Throughout much of its history, the United States was predominantly a rural society. The need to provide sustenance resulted in many people settling in areas where food could be raised for their families. Over the past century, however, a quiet shift from a rural to an urban society occurred, such that by 1920, for the first time, more members of our society lived in urban regions than in rural ones. This was made possible by changing agricultural practices. No longer must individuals raise their own food, and the number of person-hours and acreage required to produce food has steadily been decreasing because of technological advances, according to Roundtable member James Merchant of the University of Iowa.The Institute of Medicine''s Roundtable on Environmental Health Science, Research, and Medicine held a regional workshop at the University of Iowa on November 29 and 30, 2004, to look at rural environmental health issues. Iowa, with its expanse of rural land area, growing agribusiness, aging population, and increasing immigrant population, provided an opportunity to explore environmental health in a region of the country that is not as densely populated. As many workshop participants agreed, the shifting agricultural practices as the country progresses from family operations to large-scale corporate farms will have impacts on environmental health.This report describes and summarizes the participants'' presentations to the Roundtable members and the discussions that the members had with the presenters and participants at the workshop.

Nanotechnology is often described as an emerging technology - one that not only holds promise for society, but also is capable of revolutionizing our approaches to common problems. Nanotechnology is not a completely new field; however, it is only recently that discoveries in this field have advanced so far as to warrant examination of their impact upon the world around us.

Nanotechnology has direct beneficial applications for medicine and the environment, but like all technologies it may have unintended effects that can adversely impact the environment, both within the human body and within the natural ecosystem. How does the science move forward in a way that best protects the public and gets health and safety right the first time? Implications of Nanotechnology for Environmental Health Research identifies the areas in which additional research is needed and the processes by which changes can occur.

In 1992 world leaders met at the Earth Summit in Rio de Janeiro to reaffirm the Declaration of the United Nations Conference on the Human Environment that was established on June 16, 1972 in Stockholm. The meeting resulted in the adoption of Agenda 21 by the member states which is a framework for the transition to a more sustainable world. In 2012 the members gathered to assess and reaffirm the importance of progress towards the efforts of Agenda 21.

Natural gas extraction from shale formations, which includes hydraulic fracturing, is increasingly in the news as the use of extraction technologies has expanded, rural communities have been transformed seemingly overnight, public awareness has increased, and regulations have been developed. The governmental public health system, which retains primary responsibility for health, was not an early participant in discussions about shale gas extraction; thus public health is lacking critical information about environmental health impacts of these technologies and is limited in its ability to address concerns raised by regulators at the federal and state levels, communities, and workers employed in the shale gas extraction industry.

Health Impact Assessment of Shale Gas Extraction is the summary of a workshop convened in 2012 by the Institute of Medicine (IOM) Roundtable on Environmental Health Sciences, Research, and Medicine to discuss the human health impact of shale gas extraction through the lens of a health impact assessment. Eminent scientists, physicians, public health experts, and representatives from government agencies at federal and state levels, from nongovernment organizations, from the business sector, and from interest groups representing the interests of the citizens met to exchange ideas and to inform on hydraulic fracturing as a means of extraction of natural gas. This report examines the state of the science regarding shale gas extraction, the direct and indirect environmental health impacts of shale gas extraction, and the use of health impact assessment as a tool that can help decision makers identify the public health consequences of shale gas extraction.

Global Development Goals and Linkages to Health and Sustainability is the summary of a public three-part webinar series hosted by the Global Environmental Health and Sustainable Development Innovation Collaborative between October and December, 2012. Experts within the fields of environmental and global health and members of government, academia, and civil society discussed global development goals that will focus on sustainable development to inform the Rio+20 post-2015 development agenda framework, which will likely be adopted at the September 2015 UN General Assembly. This report focuses on possible health-related measures and metrics that can be utilized for creating new Sustainable Development Goals as the Millennium Development Goals sunset in 2015, using existing measurements that can be adapted to track progress of global sustainable development and human health.

Liquid fuels are a major part of modern life. They supply energy for ground, water, and air transportation as well as power for industrial and farming machinery. But fossil fuels - the dominant liquid fuel in use for well over a century - have many disadvantages. The use of fossil fuels has obvious health downsides, such as emissions of pollutants that are directly harmful to health. The burning of fossil fuels produces greenhouse gases, which contribute to global warming, itself a long-term threat to human health. There have also been health concerns related to insecurity of liquid fuel supplies and the potential of international conflicts being caused by fuel scarcity. Furthermore, there are concerns that the world''s large but still limited supply of fossil fuels could be strained by the increasing demand that results from societies around the world achieving greater prosperity. In the face of these concerns, new policies have been created that encourage the development of renewable sources of energy in general and biofuels in particular.

In January 2013, the Roundtable on Environmental Health Sciences, Research, and Medicine of the Institute of Medicine held a 2- day, interactive, public workshop on the intersection of biofuels, climate change, and human health. Workshop attendees explored public health issues related to the composition of traditional and alternative fuels and fuel additives, and they discussed the known and potential health impacts associated with the use of these fuels and fuel additives. The Nexus of Biofuels, Climate Change, and Human Health is the summary of that workshop. This report examines air, water, land use, food, and social impacts of biomass feedstock as an energy resource, and the state of the science and health policy implications of using different types (and generations) of biofuels as an energy source.

Understanding the Connections Between Coastal Waters and Ocean Ecosystem Services and Human Health discusses the connection of ecosystem services and human health. This report looks at the state of the science of the role of oceans in ensuring human health and identifies gaps and opportunities for future research. The report summarizes a workshop convened by the Institute of Medicine''s Roundtable on Environmental Health Sciences, Research, and Medicine. Participants discussed coastal waters and ocean ecosystem services in the United States in an effort to understand impacts on human health. Understanding the Connections Between Coastal Waters and Ocean Ecosystem Services and Human Health focuses on key linkages by discussing the ecosystem services provided by coastal waterways and oceans that are essential for human health and well-being; examining the major stressors that affect the ability of coastal waterways and ocean systems to provide essential services; and considering key factors that can enhance the resiliency of these systems.

Including Health in Global Frameworks for Development, Wealth, and Climate Change is the summary of a three-part public webinar convened by the Institute of Medicine Roundtable on Environmental Health Sciences, Research, and Medicine and its collaborative on Global Environmental Health and Sustainable Development. Presenters and participants discussed the role of health in measuring a country''s wealth (going beyond gross domestic product), health scenario communication, and international health goals and indicators. The workshop focused on fostering discussion across academic, government, business, and civil society sectors to make use of existing data and information that can be adapted to track progress of global sustainable development and human health. This report examines frameworks for global development goals and connections to health indicators, the role for health in the context of novel sustainable economic frameworks that go beyond gross domestic product, and scenarios to project climate change impacts.
