Vascular and endovascular procedures either in the acute phase or during intermediate or long term follow-up may fail.Re-do interventions often tend to be lengthy and difficult and are associated with an increased risk of complications. The evidence base for revision procedures is patchy compared to primary interventions. The aim of this book is to provide the specialist and the trainee with an update on the vascular and endovascular options that are available to help patients whose previous interventions have not worked.

Reveals that philosophical issues arise from real life-problems in the effort of man to conquer nature and, as such, are meaningful and exciting. What greater thrill is there than to walk along the path of great philosophers and scientists at the mountain peaks of intellectual achievement! Progress in science is always the result of overcoming our specificities - i.e. limitations - in our biologic and cultural constitution.

We live in an era in which rapid and impressive advances are being made in the field of locoregional cancer treatment, while technology simultaneously provides greater support to the surgical treatment of locally advanced cancer. The increasing interest of physicians to learn and apply these new therapeutic modalities is becoming more pronounced.

Lacan was a reminder of something fundamental. He examined human nature and human condition against any narrowly defined parameters and claimed ignorance of the self, which does not exist as a constitution, as a sum of attitudes revolving around a core, but, instead, as a borderland: it is not a stable, recognizable entity, but an open field of outside forces, false identifications, and self-deceptions, it is a fusion of inside and outside, a collage, a collection of partial views. For Lacan, there is a fundamental inadequacy-internal and integral-a biological insufficiency, a lack of being, a lack of ontological autonomy, an ontological uncertainty on which suffering is founded and which marks the ultimate tragedy of the human condition. Because one lacks innate being and wants to eradicate this loss, one identifies with persons and images, while the ego is this internalization of otherness.

There are numerous kinds of multivisceral transplant operations: e.g. all of the intra-abdominal organs, liver-duodenum-pancreas, liver-stomach-duodenum-pancreas, liver-intestine and other combinations. All of these procedures are derived from an operation described in dogs in 1960-62 in which the spleen also was part of the full multivisceral allograft. The constituent organs could be construed as grapes on a vascular stem - to be left intact, picked off and discarded, or transplanted separately. When the intestine was included in the allograft, it always has been the Achilles heel of the transplant.
