The economics of substance use and abuse deals with the consumption of goods that share two properties. First, they are addictive in the sense that an increase in past consumption of the good leads to an increase in current consumption. Second, their consumption harms the consumer and others. This second property makes them of interest from policy, legal, and public health perspectives. The tremendous expansion in research in the economics of substance use and abuse since the early 1980s and the presence of many unresolved issues motivate this volume. While most of the papers are by economists, the disciplines of medicine, political science, and psychology are also represented. Any successful attempt to address substance use must adopt an interdisciplinary perspective. The aim of the volume to cover issues pertaining to individual behavior, social interactions, markets, and politics makes this all the more necessary. Some of the twenty papers in the volume contain new estimates of the price sensitivity of alcohol, cigarettes, marijuana, cocaine, and heroin. Others focus on the effects of consumption on earnings, crime, suicide, and sexually transmitted diseases. Still others address the roles of psychobiology, social interaction, hyperbolic discounting, and peer effects in shaping decisions with regard to the use of harmfully addictive substances. To a larger or lesser extent, all the papers contain implications for policy-making. A number of papers, however, are more directly concerned with policy-making and with the policy-making environment, including evaluations of the costs and benefits of treatment services for abusers. Readers of this volume should gain a much better understanding of what we know and what we still need to know about the economics of substance use and abuse.

Obesity, which has increased in most developed countries in the past few decades, is the result of genetics, environment, and individual choices. Economics is useful for studying the individual choices that lead to obesity, explanations for the recent rise in obesity, the treatment options for obesity, and the costs and consequences of obesity for the individual and society. The papers in this volume, devoted to the economics of obesity, illustrate the wide usefulness of the economic approach. The papers in the first section propose and test economic explanations for food-consumption choices and obesity. In particular, they assess the impact of food quality, access to fast food, food prices, legislation, and other factors on diet, physical activity, and body weight. Treatments for obesity, specifically bariatric surgery and anti-obesity drugs, are studied in the second section of this volume. The third section is devoted to the labor market impacts of obesity; evidence from fifteen countries is presented and evaluated. The fourth and final section calculates the impact of obesity on hospital costs and examines the externalities imposed by obesity through health insurance. Taken together, the papers in this volume advance the frontier of knowledge about the causes, implications, and consequences of obesity, and validate the usefulness of the economic approach for studying obesity in particular and medical conditions more generally. This book studies obesity from an economic perspective. It proposes economic explanation for food consumption choices, treatment of obesity, and treatment of the condition. It's international in scope, providing evidence from 15 countries.

Hospitals worldwide command the majority of any countries health care budget. Reasons for these higher costs include the aging of the population requiring more intensive health care treatments provided in hospitals, the relatively high costs of labor in this labor intensive industry and payment systems that may encourage inefficient behavior on the part of hospital managers and physicians. Governments are seeking to instruments to mitigate this cost rise. Liberalizing hospital markets, deregulation, changing budget systems and changing ownership are only a few examples of attempts to make hospitals more efficient. Hospital industry responds in various ways to changing market conditions and legislation. In most western hospital markets we observe hospital consolidation, acquisitions, mergers and the founding of several types of network and hospital associations. The question is whether this trend also contributes to more efficiency. In this volume a number of outstanding internationally known scholars in the field of productivity measurement and health economics provide the reader with an excellent insight in the complexity of the issue. They explain that there is no straightforward panacea or recipe for the issues addressed. It is shown that the composition of the demand for care, the economic context, environmental and geographical conditions affect the outcomes. Policymakers should therefore take these nuances into account. A policy of increasing productivity starts with knowledge and insights in the complexity of the issue. The book therefore advocates the development of a strategy of collecting relevant data and conducting academic research that meet the standard of the state of the art. The book provides two illustrative examples of such a strategy in Finland and Australia. The authors have avoided as much as possible the technical jargon and complex mathematics and statistics involved in this research area. Therefore the book is par excellence suitable for policymakers and hospital managers, as well as for graduate students of health economics and health administration. It avoids, as much as possible, technical jargon and complex mathematics and statistics. It has international in scope.

Medical technology broadly defined to include all aspects of the process of treating disease (e.g., pharmaceuticals, medical devices, and surgical procedures) is profoundly important for individual health and, consequently, also for general welfare. Advances in medical technology convey the prospect of both improved population health and increased general welfare. However, because of the extensive regulation of the markets for healthcare goods and services, the development and application of medical technologies differs fundamentally from non-medical technological advances. In this volume of the "Advances in Health Economics and Health Services Research" series we present several papers that provide theoretical and empirical evidence about the market for medical technology.

Measurements of individual benefits of different health and medical interventions are fundamental for prioritizing among different alternative uses of resources in the healthcare sector. While psychometric measures do not necessarily provide information sufficient for assigning relative values to different health states, preference-based approaches produce measures that allow comparisons of such values. In this volume of the series of Advances in Health Economics and Health Services Research, entitled Preference Measurement in Health, the papers cover altruism within families, differences in risk attitudes, and estimation of health benefits of food safety. Specific topics include efficiency and altruism, comparison of mother and daughter values of HPV vaccination for daughters, differences in risk attitudes between women and men, how context matters in valuing food safety programs, and valuation of health risks associated with pesticide use.

Human capital is embodied in human beings. It embraces the individual’s capacity to perform and enjoy activities that provide money and/or psychic income. Health behaviour affects human capital and is itself affected by the individual’s human capital. This volume consists of original theoretical and empirical contributions to our knowledge of the interdependence between Human Capital and Health Behaviour.

Much of the debate about health policy in the United States has focused on the availability of health insurance coverage and the relatively large number of individuals who are uninsured. While tackling the problem of the uninsured might improve access to and utilization of health care, it would likely have little effect on the health of the population, as there is only a weak connection between health insurance coverage and health. Expanding health insurance coverage alone is unlikely to significantly improve the health of the population or narrow health disparities within the population, given that many of the major causes of poor health such as smoking, obesity, and physical inactivity are largely unaffected by health insurance. The narrow focus on the uninsured in the health policy debate comes at the expense of other policies that could improve health faster and more significantly for every dollar spent. It is well known that the United States spends approximately twice as much per capita on health care as most other developed nations, but that there is little difference in population health between the United States and other developed nations. This suggests that we are on the 'flat part of the curve' of health care spending with respect to health and as a result need to pursue other approaches for improving population health. In light of the imbalance in the health policy debate in the US, in November 2007, the Institute of Government and Public Affairs and the College of Medicine of the University of Illinois sponsored a conference entitled, Beyond Health Insurance: Public Policy to Improve Health. The purpose of the conference was to make available to the public new research on policies that can significantly improve the health of the US population. The conference focused on four areas: reducing racial and ethnic health disparities, preventing disease and promoting health, developing and regulating pharmaceuticals, and improving consumer information.

The purpose of the series "Advances in Health Economics and Health Services Research" is to consider all topics in health economics and the related field of health services research. Each volume is devoted to a specific topic. Most volumes consist of between ten and twelve papers, but a few may take the form of monographs by one or more authors. All areas in the field are covered including the production of and demand for health; the demand for medical care services; the financing of these services; the markets for physicians, nurses, dentists, hospitals, and drugs; the economics of substance use; health in developing countries; and, the economics of medical technology. The series is a forum for theoretical and empirical research. The latter employ data both for developing and developed countries. While most studies focus on economic issues, contributions from other social scientists are welcomed.

Low- and middle-income countries face major challenges to their health systems. These include a high burden of communicable disease and an emerging non-communicable disease burden. Coverage of effective services and interventions is inadequate and often constrained by funding availability. At the same time, the international financing environment is changing rapidly, with new funding streams becoming available in part as a response to the challenges of meeting the Millennium Development Goals. These countries have taken a diversity of approaches to health care financing policies and programs to face the old and emerging challenges. This is increasingly accompanied by conceptual and applied research which is contributing to our understanding of how different financing mechanisms can contribute to the overall objectives of a health care system. The goal of this volume is to assemble the best of this research and synthesize 'best practices' for the benefit of researchers, policy makers and high level administrators, dealing with all elements of health care financing and focusing on both middle- and low-income settings, to represent the experiences of all regions of the developing world.
