This book traces the theoretical history of psychosomatics in psychoanalysis, and with it the ways that psychoanalytically-trained clinicians have tried to understand and treat patients with complex psychosomatic symptoms. It offers a rethinking of the mind-body relationship in psychoanalysis, eschewing past dichotomies between the psychological and the corporeal, and today's either-or distinctions between symbolizing and non-symbolizing patients. Theoretical and clinical issues are considered from a broad and integrative perspective. Psychosomatic patients' best interests are served neither by an indiscriminate embrace of dazzling new findings, nor by discarding established ways of understanding them. This volume exemplifies an approach that takes advantage of the rich history of the past as well as exciting new work in the neurosciences. The opening historical chapter delineates the evolution of the field of psychoanalytic psychosomatics.

The premise of this book is that films, like other works of the imagination, may be elucidated by applying methods derived from psychoanalysis, and that doing so will result in a deeper and richer appreciation of the film's meaning. The book explores a number of feature films that lend themselves particularly well to this process. Both in his introduction and throughout the text, the author comments on the method and discusses continuities, similarities and differences among the films. The book is structured according to the central themes of the films, including time and death, love and lust, secrets, and human identity. Some of the films are relevant to more than one of these thematic elements. The introductory essay explores the themes, their representation in the films, and the ways in which they may be elucidated by a psychoanalytically informed critique. Brief paragraphs between the sections of the book facilitate the transitions. In an appendix, there are three essays titled 'Mise-en-scene,' 'Whatever Flames Upon the Night,' and 'Mad Doctors.'

This book examines the forces of sadomasochism in the clinical domain where transference and countertransference reside. Psychoanalysts write in depth about cases where sadomasochism is present for both analysand and analyst. Four cases present the unfolding analytic exchange where life and death forces collide. Each case is accompanied by three discussions illuminating the complex phenomena that often include lifelong perversions and painful narcissistic difficulties. Through the case presentations and discussions, psychoanalytic therapists will find maps for guiding their own work with sadomasochistic processes. Treatments where sadomasochism is prominent abound with dramas containing control and denigration, domination, and submission. Often there is a history of over stimulation and under stimulation from infancy and childhood influencing the formation of object relations and unconscious fantasy. Since Freud first introduced the concepts of component instincts and psychosexual development, psychoanalysts have been exploring sadomasochism in its various forms. The belief that togetherness involves tormenting pain creates a sense of life and death struggle that is imbued with powerful instinctual gratification. Unconscious sexualized scenes of both dyadic and triadic forms carry humiliation and conquest.

In our current professional climate, with calls for 'evidenced-based treatment', and in light of the prestige accorded to this emblem, we can ask: for what purpose do we seek evidence? For our students? For the public at large? For an inner sense of feeling supported by science? Most disciplines are concerned with cumulative knowledge, aimed toward self-affirmation and self-definition, that is, establishing a sense of legitimacy. The three parts of this volume are directed toward the goal of affirming a public and private sense of the legitimacy of psychoanalysis, thereby shaping professional identity. In each contribution we adhere to the precepts of 'scientific inquiry', with a commitment to affirming or disconfirming clinical propositions, utilizing consensually agreed upon methods of observation, and arriving at inferences that are persuasive and have the potential to move the field forward. Beyond this, each part of this book describes distinct methodologies that generate evidence pertaining to public health policy, the persuasiveness and integrity of our psychoanalytic concepts, and phenomena encountered in daily clinical practice.

This is also a clinician's book, however, aimed at helping psychoanalytic and psychodynamically-oriented clinicians to work more comfortably and productively with psychosomatic patients. The case histories are full of useful ideas about how to engage and maintain these challenging and vulnerable people in treatment. Clinicians will find many imaginative approaches to helping a patient know her own mind (often for the first time), and become capable of genuine aliveness, relatedness, and mutuality. Stalking is a predatory form of terrorizing people. The title of this book refers to a particular construction of the world that brooks no uncertainty: 'things are the way I believe them to be'. There is no other way! This can be a real boost to one's confidence - even though this conviction is based solely on our own thoughts or immediate experience. When a group or organization share a one-dimensional view of the world the sense of conviction takes the form of a rigid ideology; and all other perspectives must be eliminated.The counterpart to concreteness, or what many refer to as desymbolized thinking/experience or thing - presentations, is more abstract thinking or "symbolization". Symbolization refers to a process whereby we can meaningfully understand that an event can be looked at from a variety of perspectives. Symbolization makes it possible to look at things in an "as if" way rather than as "true" or absolute. It is a process where we can view our thoughts as objects of our thoughts. We self-reflect. From a psychoanalytic perspective the more desymbolized person might be considered unsuitable for psychoanalysis with its emphasis on self-reflection. The authors of this book disabuse us of this as they describe their work with patients who are prone to concrete thinking and experience.
