This Open Access volume provides in-depth analysis of the wide range of ethical issues associated with drug-resistant infectious diseases. Antimicrobial resistance (AMR) is widely recognized to be one of the greatest threats to global public health in coming decades; and it has thus become a major topic of discussion among leading bioethicists and scholars from related disciplines including economics, epidemiology, law, and political theory. Topics covered in this volume include responsible use of antimicrobials; control of multi-resistant hospital-acquired infections; privacy and data collection; antibiotic use in childhood and at the end of life; agricultural and veterinary sources of resistance; resistant HIV, tuberculosis, and malaria; mandatory treatment; and trade-offs between current and future generations. This book presents a comprehensive theory of the ethics and political philosophy of public health surveillance based on reciprocal obligations among surveillers, those under surveillance, and others potentially affected by surveillance practices.  Public health surveillance aims to identify emerging health trends, population health trends, treatment efficacy, and methods of health promotion--all apparently laudatory goals.  Nonetheless, as with anti-terrorism surveillance, public health surveillance raises complex questions about privacy, political liberty, and justice both of and in data use. Individuals and groups can be chilled in their personal lives, stigmatized or threatened, and used for the benefit of others when health information is wrongfully collected or used.  Transparency and openness about data use, public involvement in decisions, and just distribution of the benefits of surveillance are core elements in the justification of surveillance practices.  Understanding health surveillance practices, the concerns it raises, and how to respond to them is critical not only to ethical and trustworthy but also to publicly acceptable and ultimately sustainable surveillance practices. The book is of interest to scholars and practitioners of the ethics and politics of public health, bioethics, privacy and data technology, and health policy.  These issues are ever more pressing in pandemic times, where misinformation can travel quickly and suspicions about disease spread, treatment efficacy, and vaccine safety can have devastating public health effects.

It complements epidemiological or surveillance evidence, and reveals stakeholder perspectives crucial for public health practitioners to develop effective and ethical public health interventions. Because it relies on the natural and universal appeal of stories, the book also serves to introduce the field of public health to students considering a career in public health. The opening section of the book also serves as a more didactic introduction to public health ethics and the field of narrative ethics. It describes the field of public health ethics including ethical principles relevant to public health practice and research, and the advantages of a narrative ethics approach. That approach explores the problems and the ethical challenges of public health from the inside, from the perspective of those experiencing health problems to the challenges of those who must address these problems. The later sections consist of 14 chapters that present the actual stories of these public health problems and challenges. In narrative style they range from first person narratives of both practitioners and citizens, to analysis of published short stories. The problems and challenges they address include issues relating to justice concerns, surveillance and stigma, community values and the value of community, trust and the value of information, and freedom and responsibility. This work discusses these issues on a case to case basis and helps create awareness and understanding of the ethics of public health care. The main audience for the casebook is public health practitioners, including front-line workers, field epidemiology trainers and trainees, managers, planners, and decision makers who have an interest in learning about how to integrate ethical analysis into their day to day public health practice.  The casebook is also useful to schools of public health and public health students as well as to academic ethicists who can use the book to teach public health ethics and distinguish it from clinical and research ethics.

Given their goals and social responsibilities, influential health organizations recognize health impacts compounded by geography, social values, social determinants of health, health behaviors, and relationships between humans and environments primarily described in feminist ethics and environmental ethics. Health impacts are relevant to, but seldom addressed in bioethics, global health, public policy, or health or environmental policy. This book is the first to describe cultural, geographic, and socioeconomic factors that influence the regional significance of these impacts and frame them for bioethics and policy analyses.

This book provides an early exploration of the new field of disaster bioethics:examining the ethical issues raised by disasters. Healthcare ethics issues are addressed in the first part of this book. Large-scale casualties lead to decisions about who to treat and who to leave behind, cultural challenges, and communication ethics. The second part focuses on disaster research ethics.With the growing awareness of the need for evidence to guide disaster preparedness and response, more research is being conducted in disasters. Any research involving humans raises ethical questions and requires appropriate regulation and oversight. The authors explore how disaster research can take account of survivors? vulnerability, informed consent, the sudden onset of disasters, and other ethical issues. Both parts examine ethical challenges where seeking to do good, harm can be done. Faced with overwhelming needs and scarce resources, no good solution may be apparent. But choosing the less wrong option can have a high price. In addition, what might seem right at home may not be seen to be right elsewhere. This book provides in-depth and practical reflection on these and other challenging ethical questions arising during disasters.Scholars and practitioners who gathered at the Brocher Foundation in Geneva, Switzerland in 2011 offer their reflections to promote further dialogue so that those devastated by disasters are respected by being treated in the most ethically sound ways possible.
