Coronary heart disease is the single most common cause of death in the UK. Much of the ensuing morbidity and premature morbidity is preventable and the Government is addressing this with the National Service Framework (NSF) initiative. This is a practical text that can be used by an individual practice team or by a group of practices working together. It gives examples of personal development plans and a practice learning plan showing how the information can be applied by a GP health professional or practice team. Topics such as heart failure smoking cessation and primary and secondary prevention of coronary heart disease are included as well as appendices providing NSF standards and goals Read codes and further useful information.
