The growing importance of the evidence-based movement has made experimental evaluation a key issue among researchers, practitioners, commissioners and policy makers. However, experimental evaluation remains controversial in the sexual health field. This partly reflects the diversity of groups involved in this area and their different views on the most appropriate research methods. This book provides an analysis of the methodological and practical issues involved in evaluating sexual health interventions. The book will appeal to trial enthusiasts through discussion of specific issues in trial design, and also to those with a sceptical interest in the potential of experimentation and its appropriateness or feasibility. It is concerned with methodology rather than the substantive findings of research, and considers the requirements of research in both developed and developing countries. The focus of the book is on sexual health interventions, although many of the issues are equally applicable to other areas of behavioural and social research

This book describes an innovative approach to the evaluation of complex health interventions, assessing what interventions work, how and for whom. Rejecting the stalemate between trials and realist evaluation, it draws on the best of both. Randomised controlled trials and systematic reviews offer the least biased means of assessing intervention effects but tell us little scientifically about how interventions work. Policy-makers and practitioners are also not supported to decide which interventions are likely to achieve most benefits in their local contexts. Realists use other forms of evaluation and evidence synthesis exploring how intervention mechanisms interact with context to generate outcomes. But these approaches lack rigour in assessing causality. This book proposes how realist evaluation methods may be incorporated within randomised controlled trials and systematic reviews. This enables evaluations and evidence synthesis to benefit from the more nuanced questions posed within realist enquiry while maintaining rigour in assessing causality.

Young people’s mental health is in crisis, with many – especially those from disadvantaged backgrounds – struggling academically and with the later transition to employment. Feeling excluded, many young people turn to harmful behaviours, such as vaping and alcohol use, for escape and a sense of belonging. Schools are increasingly expected to address these issues but often lack the time and expertise to do so effectively. Based on the author’s research, including the successful ‘Learning Together’ trial – an innovative programme that improved mental health, reduced bullying and raised academic achievement – this book provides a blueprint for a fundamental shift in how schools support young people.Essential reading for teachers, public health workers and policy makers tackling the health and educational inequalities affecting young people today.
