In recent years, substantial efforts have been initiated to develop new drugs, vaccines, and other medical interventions against biological agents that could be usedin bioterrorist attacks against civilian populations. According to a new congressionallymandated report from the Institute of Medicine and National Research Council ofthe National Academies, to successfully develop these drugs, vaccines, and othermedical interventions against biowarfare agents, Congress should authorize the creationof a new agency within the Office of the Secretary of the U.S. Department ofDefense. The committee recommended that Congress should improve liability protectionsfor those who develop and manufacture these products, to stimulate willingnessto invest in new research and development for biowarfare protection. GivingFull Measure to Countermeasures also identifies other challenges—such as the needfor appropriate animal models and laboratories equipped with high-level biosafetyprotections—that will require attention if DoD efforts to develop new medical countermeasuresare to be successful.

There is growing interest in using performance measurement as a means of holding federal, state, and local health agencies accountable for their use of public funds. Health Performance Measurement in the Public Sector is the second of two books for the U.S. Department of Health and Human Services on using and improving performance measurement in publicly funded health programs and the implications for data needs and systems.
This book focuses on data and information system issues at the federal, state, and local levels. Recommendations address:
Policy framework for selecting performance measures and using performance measurement. Operational principles related to data and data systems that support performance measurement. Essential investments in data systems and in training and technical assistance. Research needed to improve performance measures and performance measurement.1 047 kr

Vaccines have made it possible to eradicate the scourge of smallpox, promise the same for polio, and have profoundly reduced the threat posed by other diseases such as whooping cough, measles, and meningitis.
What is next? There are many pathogens, autoimmune diseases, and cancers that may be promising targets for vaccine research and development.
This volume provides an analytic framework and quantitative model for evaluating disease conditions that can be applied by those setting priorities for vaccine development over the coming decades. The committee describes an approach for comparing potential new vaccines based on their impact on morbidity and mortality and on the costs of both health care and vaccine development. The book examines:
Lessons to be learned from the polio experience. Scientific advances that set the stage for new vaccines. Factors that affect how vaccines are used in the population. Value judgments and ethical questions raised by comparison of health needs and benefits. The committee provides a way to compare different forms of illness and set vaccine priorities without assigning a monetary value to lives. Their recommendations will be important to anyone involved in science policy and public health planning: policymakers, regulators, health care providers, vaccine manufacturers, and researchers.

How do communities protect and improve the health of their populations? Health care is part of the answer but so are environmental protections, social and educational services, adequate nutrition, and a host of other activities.
With concern over funding constraints, making sure such activities are efficient and effective is becoming a high priority.
Improving Health in the Community explains how population-based performance monitoring programs can help communities point their efforts in the right direction.
Within a broad definition of community health, the committee addresses factors surrounding the implementation of performance monitoring and explores the "why" and "how to" of establishing mechanisms to monitor the performance of those who can influence community health. The book offers a policy framework, applies a multidimensional model of the determinants of health, and provides sets of prototype performance indicators for specific health issues.
Improving Health in the Community presents an attainable vision of a process that can achieve community-wide health benefits.

Many individuals, groups, and federal agencies have a strong interest in finding answers to the numerous and complex questions regarding the health of Gulf War veterans. Various types of research and health measurement are needed to address these diverse issues. The Institute of Medicine (IOM) was asked by the Department of Veterans Affairs (VA) and the Department of Defense (DoD) to undertake a study to identify important questions concerning the health of Gulf War veterans and then to design a study to answer those questions. The committee determined that it is of fundamental importance to ask how healthy are Gulf War veterans? Are they as healthy as others? What characteristics are associated with differences between the health of Gulf War veterans and the health of others?
To address these questions, it will be necessary to measure not only the health status of those who served in the Gulf War, but also to compare Gulf War veterans with other groups. Further, one must continue to follow these groups through time to determine whether the groups differ in the way their health status is changing. As the committee began to develop a design that would address the fundamental questions identified, it realized that such a study could have important implications for understanding not only the health of Gulf War veterans, but also the health of veterans of other conflicts.

How can we meet the special needs of children for emergency medical services (EMS) when today''s EMS systems are often unprepared for the challenge? This comprehensive overview of EMS for children (EMS-C) provides an answer by presenting a vision for tomorrow''s EMS-C system and practical recommendations for attaining it.
Drawing on many studies and examples, the volume explores why emergency care for children—from infants through adolescents—must differ from that for adults and describes what seriously ill or injured children generally experience in today''s EMS systems.
The book points the way to integrating EMS-C into current emergency programs and into broader aspects of health care for children. It gives recommendations for ensuring access to emergency care through the 9-1-1 system; training health professionals, from paramedics to physicians; educating the public; providing proper equipment, protocols, and referral systems; improving communications among EMS-C providers; enhancing data resources and expanding research efforts; and stimulating and supporting leadership in EMS-C at the federal and state levels.
For those already deeply involved in EMS efforts, this volume is a convenient, up-to-date, and comprehensive source of information and ideas. More importantly, for anyone interested in improving the emergency services available to children—emergency care professionals from emergency medical technicians to nurses to physicians, hospital and EMS administrators, public officials, health educators, children''s advocacy groups, concerned parents and other responsible adults—this timely volume provides a realistic plan for action to link EMS-C system components into a workable structure that will better serve all of the nation''s children.
