Vor 15 Jahren war der Erstautor dieser Zeilen als Assistenzarzt Chronist der ersten in Deutschland durchgeführten Mammarekonstruktionen. Aufwendige Verfahren mit Rollstielplastiken aus dem Unterbauch führ­ ten nach multiplen Eingriffen zu ersten Erfolgen. Die von den Frauen akzeptierten Belastungen lehrten uns den Leidensdruck der Betroffenen zu wägen und die Bedeutung verstümmelnder Eingriffe auf die Psyche der Frau einzuschätzen. In diesen 15 Jahren hat sich vieles verändert. Die operativen Verfah­ ren wurden standardisiert, die Angst um das Lokalrezidiv gebannt und der Zeitpunkt der Rekonstruktion neu definiert. 1981 wurden in den USA bei 20000 Frauen Rekonstruktionen vorgenommen, 1984 waren es bereits 98 000. Verläßliche Zahlen aus dem deutschen Sprachraum liegen nicht vor, aber die Tendenz ist ähnlich. Anfänglich wurden nur frühe Tumorstadien mit guter Prognose nach 1-2 Jahren Wartezeit operiert. Heute weiß man, daß die Mamma­ rekonstruktion bei korrekter Technik kein Lokalrezidiv verschleiert. Zu­ nehmend rückt die Sofortrekonstruktion bei der Ablatio in den Vorder­ grund. Im Extrem kann sogar eine Patientin mit vorliegender Metasta­ sierung direkt mit einer Rekonstruktion versorgt werden, um ihr für die letzten verbleibenden Monate oder Jahre eine bessere Lebensqualität anzubieten. Sehr fortgeschrittene Primärtumoren und das inflammato­ rische Mammakarzinom sind einzige Ausschlußkriterien. Beim Vergleich zwischen "mastektomierten" und "rekonstruierten" Frauen findet man eine deutlich verringerte Morbidität bei den letzteren.
