I wrote the first edition of this work in 1987 with the object of providing a review of the aetiology, pathogenesis and morphological basis of diseases of the ear, nose and throat. Since that time important changes have taken place in the disciplines of both histopathology and otorhinolaryngology. Pathology has shifted its research orientation towards a molecular one; notable successes with new medical and surgical treatments in ENT have resulted in a striking expansion of that field. To provide a precis of the scientific principles of a subject lying between two such mobile territories was a daunt ing prospect. Help was indeed necessary and I welcome Henrik Hellquist as the co author of this second edition. He has had a longstanding interest in both head and neck and molecular pathology and has not only produced scholarly publications, but has also played an important role in enhancing international cooperation in this combined field.

In their preface, the authors highlight the great social and medical importance of hearing loss in the elderly, an ever-increasing prob lem. It is, therefore, most appropriate that a monograph, devoted to research in this field, features as an addition to the Bloomsbury Series in Clinical Science. Written by two leading authorities, the book reviews the past and present literature, details the clinical aspects and considers the electrophysiological and histopatho logical issues basic to the overall problem. It continues the high standard and excellence fostered by previous issues in the Series. Only by a greater scientific knowledge of old age deafness can there be any possibility for improvement in the care offered to its sufferers.

In this work I endeavour to explain the morphological changes in diseases of the human ear, nose, larynx and pharynx. Efficient ENT practice, like that in other surgical specialties, relies on close collaboration between surgeon and pathologist through the medium of biopsies. Accordingly much of the present volume emphasizes diagnostic surgical pathology. The role of the pathologist is, however, a broader one-in otolaryngology as in other specialties-contributing as well to an understanding of the disease processes. For this pur pose autopsy study is important and nowhere more so than in the disease of the inner ear, an area which has been developed mainly by otologists, but neglected by pathologists. It is hoped that, by displaying the interest and potential for development of such fields, this book may stimulate some pathologists to work in them. I kept in mind while writing the possibility that it might also be useful to ENT clinicians as a review of the pathology of their specialty.
