Delirium is a complex syndrome with a multifactorial aetiology and is characterized by marked disturbances of consciousness, attention, memory, perception, thought, sleep-wake cycle, and by fluctuation of symptoms. This book covers in detail the pathophysiology, epidemiology, clinical aspects, differential diagnosis, and management of the syndrome.Due to the special characteristics of the syndrome, specific chapters deal with different aetiologies and populations at risk, with emphasis on the critically ill and palliative care patients. As delirium often announces or anticipates the proximity of death, family issues are considered in a comprehensive final chapter, covering the impact of terminal illness on the family and the process of bereavement. The book emphasises the need for assessing and diagnosing delirium with reliable instruments, and a chapter on assessment is reinforced by including appendices of many of the most relevant instruments reported in recent literature. The evidence from the literature is always distinguished from the authors' opinions and most chapters are integrated by the presentation of case examples. Updated for the second edition this book contains new material on topics including classification systems, more data on populations at risk, and significant new material on the family and bereavement.Delirium: Acute confusional states in palliative medicine, Second Edition demonstrates that only an interdisciplinary treatment of delirium between neurology, psychiatry and palliative medicine can develop knowledge of the syndrome and improve patient and family care. This book has been written for palliative care physicians and specialist nurses, neurologists, psychiatrists, and other health professionals treating terminally ill patients, offering them a clear account of how to recognize and deal with the syndrome.

The level of psychological distress and the ability to adjust to a diagnosis of cancer are highly variable. Medical factors, psychological factors prior to diagnosis and social factors account for this variability. By understanding these variables, the clinician can better assess and manage the distress caused by the diagnosis and provide the most appropriate medical treatment or psychological intervention. This practical handbook will address the principal behavioural and psychological problems associated with cancer. Where appropriate, it adopts a broader, multicultural perspective, in line with the aim of the World Psychiatric Association and the Federation of Psycho-Oncology societies. The main aims of the book are: to present the significant and challenging clinical problems encountered when caring for cancer patients and their families, including assessment, diagnosis and treatmentto describe the best responses to these challenges, summarizing the evidence base and digesting clinical experience where evidence from clinical trials is lackingto discuss the emerging themes in psycho-oncology, such as genetic counselling, bioethics, cultural issues and cultural diversityto provide practical suggestions for dealing with special populations, such as children, the elderly, long-term survivors or mentally ill patients. The book is designed to be easy to read and to reference, with information clearly displayed in concise tables and boxes accompanied by further detail within the text.Chapters feature clinical vignettes, including management algorithmsKey Points Suggested further readingThe editors aim to provide an indispensable tool for junior doctors in training in either psychiatry, psychology or oncology, general practitioners, community psychiatric nurses, palliative care physicians and other members of the multidisciplinary team. With a Foreword by the pioneer in psycho-oncology, Professor Jimmie Holland.

This book presents a comprehensive portrait of trust in medicine and oncology. Trust is vital to medical care. It is linked to treatment recommendation adherence, continuity of care, and even treatment efficacy under certain scenarios, and is distinct from other commonly measured patient-reported outcomes that are used to drive medical practice, such as patient satisfaction. Cancer care highlights many of the issues with trust that all clinicians confront and brings to light the interplay of technology, rapidly advancing treatments, and readily available information consumed by patients and families that may complicate clinician-patient communication. Each chapter highlights a different aspect of trust in the medical setting, many of which are specific to oncology. For example, trust is fundamental to the first encounter with an oncologist. The anatomy of that encounter is explored. The elements of trust are investigated, from the institutional or fiduciary perspective to thenuances of the personal and individual encounters with patients. Several key factors are relevant to trust within these encounters, such as the patient’s background, attachment style, and perception of ethically responsible commitment. Further chapters cover topics like the proliferation of false information, health inequities, and patient-centered care. The Complex Role of Patient Trust in Oncology will be of great interest to oncologists, psychiatrists, internists, and any healthcare professional interested in the role of trust in patient care.

This book offers a resource to aid in implementing psychosocial screening, assessment, and consequently integrating prevention, care and treatment (i.e. pharmacological, psychosocial rehabilitation and psychotherapeutic) in medicine. It is becoming increasingly recognized that one method of combating spiraling health care costs in developed nations is to integrate psychiatric care into medicine including primary care settings. This volume reviews the main issues relative to the paradigm of a person-centered and recovery-oriented approach that should imbue all medical areas and specialties. It proposes integration methods in screening and assessment, clinimetric approach, dignity conserving care, cross-cultural and ethical aspects, treatment and training as a basic and mandatory need of a whole psychosomatic approach bridging the several specialties in medicine. As such, the book addresses a topic that all physicians, including primary care and psychiatric professionals in a wide variety of mental health settings are currently discussing, planning and preoccupied with, namely the task of integrating mental health into all the medical fields, including primary care, cardiology, psychiatry, oncology and so on.
