Despite sustained efforts to improve patient safety, preventable harm remains a persistent challenge in healthcare. Panagioti's 2019 meta-analytic indicate a 6% prevalence of avoidable patient harm, with 12% of these events resulting in severe outcomes or mortality. Progress in general has been limited, and a paradigm shift is needed. This second edition of Practical Patient Safety shows frontline doctors, nurses, and other healthcare professionals how techniques developed and used by staff in High Reliability Organisations, such as the railway and aviation industries. It builds on the foundational aim of its predecessor, which is to equip individual healthcare professionals with practical, evidence-based techniques for error prevention in routine clinical settings. Drawing on insights from high-reliability industries such as aviation and rail, the book explores how small, intentional changes in communication, crisis management, and workplace civility can disrupt error chains and enhance safety. With a focus on actionable strategies as structured readback, precise language use, and simulation-based preparation, this title provides a basic advice for the individual health care worker on error prevention in their day-to-day practice.

This book, a selection of folk tales, true tales, tall tales, myths, gossip, legends and memories, celebrates and honours unique Welsh stories. Some are well known, others from forgotten manuscripts or out-of-print volumes, and some are contemporary oral tales. They reflect the diverse tradition of storytelling, and the many meanings of ‘chwedlau’. If someone says, ‘Chwedl Cymraeg?’ they are asking, ‘Do you speak Welsh?’ and ‘Do you tell a tale in Welsh?’ Here is the root of storytelling, or ‘chwedleua’, in Wales. It is part of conversation. This book, one to linger over and to treasure, keeps these ancient tales alive by retelling them for a new audience.

A lone man wanders from swamp to swamp searching for himself, a wolf-girl visits Wales and eats the sheep, a Welsh criminal marries an 'Indian Princess', Lakota men re-enact the Wounded Knee Massacre in Cardiff and, all the while, mountain women practise Appalachian hoodoo, native healing and Welsh witchcraft. These stories are a mixture of true tales, tall tales and folk tales, that tell of the lives of migrants who left Wales and settled in America, of the native and enslaved people who had long been living there, and those curious travellers who returned to find their roots in the old country. They were explorers, miners, dreamers, hobos, tourists, farmers, radicals, showmen, sailors, soldiers, witches, warriors, poets, preachers, prospectors, political dissidents, social reformers, and wayfaring strangers. The Cherokee called them: 'the Moon-Eyed People'.

We are delighted to announce that this book has been short listed for the prestigious Michael Ramsey prize for the best in theological writing. For more information please visit: www.michaelramseyprize.org.uk   Preaching the Atonement explores some classic texts which throw light on the atonement. Each chapter focuses on a key Biblical passage, offering a theological commentary which will help both preachers and church members gain a deeper understanding of the atoning work of Christ. Each chapter contains a sermon on the chosen text, illustrating how these themes may be communicated to contemporary congregations. Seeking to be sensitive to the literary forms of the chosen texts, these sermons employ a range of homiletical strategies. The sermons are drawn from the works of preachers on both sides of the Atlantic. Preaching the Atonement assumes that the preacher's role is not to squeeze every passage of Scripture into the same mould. Each passage bears witness to another aspect of the many splendoured work of Christ; as the preacher invites the congregation to listen to the testimony of this cloud of biblical witnesses.

The Grimms called them The Quiet Folk, in Māori they are Patupaiarehe, in Wales Y Tylwyth Teg: hidden people who live unseen, speak their own languages and move around like migrants, shrouded from our eyes – like those who lived in the utopian world of Plant Rhys Ddwfn off the west Welsh coast, where this book begins. In mythology, lost lands are coral castles beneath the sea, ancient forests where spirits live, and mountain swamps where trolls lurk. Strip away the mythology, and they become valleys and villages flooded to provide drinking water to neighbouring kingdoms, campsites where travellers are told they can’t travel, and reservations where the rights of first nations people are ignored. The folk tales in this book tell of these lost lands and hidden people, remembered through migrations, dreams and memories.

Ceredigion is a land shaped by mythology, where mermaids and magic mix with humans and where ordinary people achieve extraordinary things. This is a captivating collection of traditional and modern stories, including the submerged city of Cantre’r Gwaelod, or the ‘Welsh Atlantis’, how the Devil came to build a bridge over the Rheidol, the elephant that died in Tregaron, and how the Holy Grail came to Nanteos. All the while the tylwyth teg (the Welsh fairies) and changelings run riot through the countryside. Storyteller and illustrator Peter Stevenson takes us on a tour of a county steeped in legend, encountering ghosts, witches and heroes at every turn.

Storyteller is storïwr in Welsh. The old word is cyfarwydd. And this book is a sneaky peek into a storyteller's repertoire.Meet the rowdy mermaids and drowned lands of Cardigan Bay, an ancient tree with a door into the otherworld, and the wise old toad who lives in Borth bog and knows everything. Discover a clever girl who transforms into a swan, the herd of fairy cattle who live beneath Llyn Barfog and an elephant who may or may not have died in Tregaron.

Cyfarwydd yw’r hen enw am storïwr, un sy’n adrodd storïau.Yn y llyfr hwn cewch gipolwg ar ei stôr o storïau gwych.Dewch i gwrdd â môr-forynion swnllyd Bae Ceredigion, gwledydd cudd dan y môr, hen goeden lle mae drws i’r byd arall, a’r llyffant doeth holl-wybodus sy’n byw yng Nghors Fochno.Neu beth am y ferch glyfar drodd yn alarch, gyr o wartheg swyn sy’n byw dan Lyn Barfog, a’r eliffant a fu farw – efallai – yn Nhregaron?

Just ask those mountains over there. They have heard these stories before. They have witnessed the floods.Through the travels of a Mesolithic Marsh Girl, Peter Stevenson tells the folk tales of the sea along the Welsh Coast Path. From bridge-dwelling bwbach and drunken mermaids, to swan girls, salmon children and the pirate Leekie Porridge of Tenby – follow Môrwen on a journey that blends past and present and explores the social and environmental change that is carved into the Welsh coastline.

Folk tales flow around the world like rivers, spilling over into imprints where history has left its mark; where cultures have been uprooted, seabeds scoured and villages drowned (but never forgotten).Follow the current out from Welsh waters, where all these stories begin, into the vast ocean. Here be sea monsters and swan girls, displaced people and rain-makers, toilet demons and talking catfish – so come on in, the water's fine.
