By the nature of their jobs, first responders are often highly exposed to traumatic events.Police, firefighters, and paramedics experience markedly higher rates of PTSD than others in the community. They also face distinct organizational challenges that complicate their stress reactions. Often, this results in needs that are not addressed in manuals developed for people who have survived single traumatic events.This book provides an overview of theoretical and empirical frameworks for understanding PTSD in first responders and outlines practical and evidence-based approaches to assess and treat PTSD in these populations. Nach belastenden Ereignissen, z.B. einem Verkehrsunfall, körperlicher bzw. sexueller Gewalt oder gar einer Großschadens- und Katastrophenlage, entwickelt ein Teil der Betroffenen eine akute Belastungsstörung. Allerdings werden die Betroffenen häufig nicht erkannt und ausreichend versorgt. Um die psychosoziale Notfallversorgung von Opfern, Angehörigen und Einsatzkräften zu verbessern, wurde dieses Manual entwickelt. Es stellt zunächst präventiv angelegte Versorgungskonzepte vor, die international empfohlene psychosoziale Interventionen berücksichtigen und erläutert anschließend praxisorientiert ein sieben Sitzungen umfassendes Programm zur kognitiv-behavioralen Behandlung der akuten Belastungsstörung.Ziel der traumafokussierenden Behandlung der akuten Belastungsstörung ist es, zwei bis drei Wochen nach dem traumatischen Ereignis in kurzer Zeit die aufgetretenen Beschwerden zu normalisieren, ein individuelles Störungsverständnis mit dem Betroffenen zu entwickeln, Intrusionen, Vermeidungsverhalten und Übererregung zu reduzieren, negative Bewertungen und Interpretationen zu modifizieren und die Anwendung der gelernten Fertigkeiten im Selbstmanagement zu fördern. Konkrete Handlungsanleitungen, zahlreiche Fallbeispiele und wörtliche Dialoge veranschaulichen das therapeutische Vorgehen. Zudem werden Hinweise gegeben, wie bei extremer Traumatisierung und bei posttraumatischen aufgetretenen Emotionen wie Wut, Scham, Schuld und Ekel vorgegangen und wie der Einbezug von Angehörigen gestaltet werden kann. Der Umgang mit Hindernissen vor und während der konkreten Umsetzung der einzelnen Behandlungsschritte wird praxisorientiert erörtert. Diagnostische Instrumente und Therapiematerialien, die zusätzlich auf einer CD-ROM vorliegen, bieten konkrete Hilfestellungen bei der einzelfallbezogenen Erfassung der Beschwerden und der anschließenden Behandlung.
