This is the perfect pharmacology textbook for medical and pharmacy students. The book was developed on the +30-year experience of the author as pharmacology professor in the United States and Germany. The book discusses the most important drugs (400) in the context of relevant diseases. Written by an experienced and well-known textbook author, this book provides an overview of the most common diseases and the drugs used to treat them.

The cyclic purine nucleotides 3’,5’-cAMP and 3’,5’-cGMP are well-established second messengers. cGMP has recently been covered in a volume of the Handbook of Experimental Pharmacology (volume 191). In addition to 3’,5’-cAMP and 3’,5’-cGMP, so-called non-canonical cyclic nucleotides exist. These comprise the cyclic pyrimidine nucleotides 3’,5’-cCMP and 3’,5’-cUMP, the purine nucleotide 3’,5’-cIMP, the 2’,3’-nucleoside monophosphates and cyclic dinucleotides. In this volume of the Handbook of Pharmacology, word-leading experts in the field summarize our current knowledge on these non-canonical cyclic nucleotides, discuss open questions, future research directions and the pharmacotherapeutic implications. Special emphasis will be given to the emerging roles of 3’,5’-cCMP and 3’,5’-cUMP as second messengers with regard to generators, effectors, biological functions, inactivation and bacterial toxins. The role of 3’,5’-cIMP as potential second messenger will also be critically discussed. Furthermore, we will consider transport of cyclic nucleotides and their potential role as first messengers. The role of the cyclic dinucleotide cGAMP in the immune system will covered, too. While the editors realize that it is almost impossible to cover the field completely within the constraints of a single HEP volume, nonetheless, all important aspects will be covered in one way or the other. An overarching introductory chapter will link the individual chapters and provide an overview on the field. This chapter will also link the book to the previous HEP volume on histamine receptors and the recent HEP volume on the pharmacology of itch. Great attention will be paid to complementation of existing literature while avoiding undue duplication. The book will cover new methods for analysis of histamine and histamine metabolites, development of methods for histamine receptor analysis, signal transduction, histamine release, regulation of immune cells by histamine, histamine metabolism and associated diseases, regulation of major organ systems by histamine and development of new drugs and experimental tools for the study of histamine receptors.

Basis sind die Verordnungsdaten von Arzneimitteln für ambulante Patienten der gesetzlichen Krankenversicherung (GKV). Datenbasis des Jahres 2020 sind ca. 820 Mio. Verordnungen von 150.363 Vertragsärzten und 62.876 Vertragszahnärzten für 73,4 Mio. GKV-Versicherte. Die allgemeine Verordnungs- und Marktentwicklung wird in 6 Kapiteln behandelt, in denen u.a. neue Arzneimittel des Jahres 2020, die Kosten-Nutzen-Analyse von onkologischen Arzneimitteln, Marktsteuerungsmechanismen und Preise von Biologika bzw. Biosimilars sowie die Klassifizierung von Arzneistoffgruppen diskutiert werden.Die Nettokosten für Arzneimittel in der GKV sind 2020 weiter um 5 % auf rund 46 Mrd. € gestiegen, vor allem durch neue, hochpreisige Patentarzneimittel. Hauptkostentreiber waren Onkologika (9,5 Mrd. €, + 12,4 %)und Antidiabetika (2,8 Mrd. €, + 16,8 %). Ein wesentlicher Grund für die Kostenprobleme ist der seit Jahren beobachtete Anstieg der Jahrestherapiekosten neu eingeführter Arzneimittel. Dieser Trend zu hochpreisigen Patentarzneimitteln ist durch das Arzneimittelmarktneuordnungsgesetz (AMNOG) sogar 10 Jahre nach seiner Einführung nicht genügend abgeschwächt worden, auch wenn mit der Vereinbarung von Erstattungsbeträgen 2020 insgesamt 3,7 Mrd. € an Einsparungen erzielt werden konnten.
