Combined positron emission tomography (PET) or single photon emission computed tomography (SPECT) with computed tomography (CT) has developed into the faste- growing imaging modality largely because combined PET or SPECT with CT data acquisition is highly synergistic in diagnosis and therapeutic evaluation. All currently available data indicate that integrated PET or SPECT/CT is more sensitive and s- ci?c than either of its constituent imaging methods alone and probably more so than images obtained from separate systems viewed side by side. The PET or SPECT/CT provides precise localization of the lesions, thereby increasing diagnostic speci?city particularly by reducing false-positive ?ndings. The other advantage of adding CT is that the transmission data obtained with the CT component is useful for attenuation correction of the emission data. This makes PET/CT 25%–30% faster than PET alone with standard attenuation–correction method, leading to higher patient throughput and a more comfortable examination, which typically lasts 30 minutes or less. With hybrid imaging, unique physiologic information bene?ts from a precise to- graphic localization. Simultaneous evaluation of metabolic and anatomic information about normal and disease processes is needed to answer complex clinical questions and also raise the level of con?dence of scan interpretation. It is always dif?cult to consider three dimensions in the mind’s eye and view the relationship of the viscera and fascial planes in transverse and vertical sections of the body’s structure. The introduction of modern imaging techniques has enormously expanded the already considerable importance of sectional anatomy.

PET has been a valuable research tool in academic institutions since the '70s, but its move into clinical practice in community hospitals has just begun. PET has undergone spectacular growth in the fields of nuclear medicine, radiology, and oncology. The burgeoning world of PET is reflected in standing room only CME courses at scientific meetings such as the Radiology Society of North America and the Society for Nuclear Medicine. This book will provide nuclear medicine practitioners, radiologists, oncologists, and neurologists with a practical overview of the basic principles and clinical applications of PET. Emphasis is placed on the familiarization of normal distribution, artefacts, and common imaging agents such as FDG in conjunction with CT, MRI, and US to establish the clinical effectiveness of PET. Practical understanding of updated PET scanners, image process and quantification of PET measurements is also discussed. With contributions from leaders in the PET community, the book deals with the basic principles, instrumentation, fusion, radiopharmaceuticals, radiosynthesis, safety and cost analysis of PET. The clinical section of the book will focus on the technique and indications of PET. There is also a unique atlas as well as comprehensive coverage of essential clinical PET studies in neurology, cardiology, and oncology.

This comprehensive volume covers radiopharmaceuticals developed for pathway-directed systems in imaging and theranostic applications. We now are at the cutting edge of providing personalized treatment with increased use in oncology of these new radiopharmaceuticals. Trends in high-resolution instrumentation development, quality assurance systems and regulatory compliance for radiopharmaceuticals, clinical evaluation of radiopharmaceuticals, and benefits and pitfalls of the current clinical FDG PET are discussed.Radiopharmaceuticals are used for diagnosis of diseases of the central nervous and cardiovascular systems and for staging, restaging, and treatment planning for cancers. Nuclear biomarkers allow precise measurement of molecular pathways on a whole-body image upon administration of functional radiolabeled agents, and nuclear imaging agentshave potential use in patient selection, pharmacokinetic, dosage-finding, and proof-of-concept studies. Nuclear imaging agents and hybrid instrumentation also provide sensitive and specific answers for differential responsiveness in therapeutic outcome. This book serves as a reference for moving the discovery and development of radiopharmaceuticals from the workbench to clinical applications. It thus benefits not only clinicians but also translational research scientists—molecular biologists, chemists, imaging scientists, pharmaceutical developers, physicists, and support staff.
