D.G.JULIAN Cardiology has been advancing on a broad front and in recent years we have learned much about the basic mechanisms underlying heart disease, and developed many new methods of diagnosis and treatment. This book discusses in depth some of the most important aspects of these. One of the most exciting areas of research has been in molecular biology; Tybjrerg-Hansen and Humphries describe how, following the pioneering Nobel prize-winning work of Goldstein and Brown, gene probes are being used to discover the genetic causes of coronary artery disease, especially in the hyperlipidaemias but also in thrombotic states. These developments, together with the introduction of powerful lipid-lowering agents has triggered interest in the primary hyperlipidaemias, which are frequently hereditary. Betteridge discusses their diagnosis and management. Quyyumi describes how a greater knowledge of the physiology and pathology of the coronary circulation has led to a better understanding of the causes of angina pectoris and of 'silent ischaemia'. Crean deals with the difficult topic of unstable angina, which has caused a lot of controversy in the past, but whose investigation and management is now broadly agreed.

We were particularly pleased to compile this volume entitled 'Clinical Aspects of Cardiac Arrhythmias'. Recent years have seen the publication of many textbooks on cardiac arrhythmias, some of which concentrate on one particular aspect such as drug management, electrocardiographic appearances, electrophysiological evaluation etc; and others of which are the collated reports of symposia, often dealing with detailed con siderations of highly specialised problems. Most of the larger more comprehensive texts have devoted a substantial proportion to basic considerations and experimental observations, far removed from the clinical arena. When asked to contribute to the series 'Current Status of Clinical Cardiology' we felt that the clinical aspects of cardiac arrhyth mias should be emphasized, and that the text should be as comprehensive as possible within the limitations of a single volume in this series. This comprehensive but clinical approach has necessitated the inclusion of certain subjects such as the mechanisms of tachycardia, metabolic aspects of cardiac arrhythmias and reperfusion arrhythmias, which are not directly or exclusively clinical. However, the rapid advances in these areas in recent years are likely to have increasingly important clinical con seq uences. Although the epidemiology of clinical arrhythmias is difficult to dis cover, it is widely appreciated that arrhythmias are commonplace.

This book attempts to focus on the most important clinical aspects of ischaemic heart disease. Clearly, in no way is it supposed to be a comprehensive review of the subject, but it does attempt to address the most important topics for everyday practising clinicians. The question of epidemiology is topical both in the lay and medical press and is, therefore, covered in some detail. It is the editor's opinion that proper understanding of the clinical manifestations of the disease and, therefore, subsequent investigation and treatment are based on a detailed understanding of the pathology and pathophysiology, and as a result these subjects have also been covered in detail. In the last decade there have been considerable advances in our understanding of the mechanisms of angina and also the haemodynamic and metabolic consequences of both angina and myocardial infarction. Again, if appropriate investigations and treatment are to be undertaken, they can only be done with the proper knowledge of the disease being investigated. Finally, the investigation and management of the various mani festations of ischaemic heart disease have been addressed. In the last ten years there has been an explosion in the drugs available for the treatment of ischaemic heart disease and how they should be used has become much more sophisticated. The place of coronary artery surgery has now been rationalized and angioplasty provides an important adjunct to treatment.

Twenty five years ago, cardiomyopathies or myocardopathies as they were sometimes called, were in very small print, and often the terms myocarditis and cardiomyopathy were used interchangeably. Now definition and classifi cation can be precise and terminology has been refined. Although a great deal still has to be learnt about the heart muscle diseases, they have now achieved the status of an important group of cardiovascular disorders. Their importance is out of proportion to their frequency because the cardiomyopathies so often attack young otherwise active and healthy people, and are notable for sudden unexpected death, and for intractable congestive heart failure. They are especially a meance in age groups younger than those most commonly effected by coronary heart disease. This book presents an analysis and review by many experts of the present knowledge about heart muscle diseases and employs the approach to classi fication and terminology now generally, though not universally, agreed. It will be apparent that much fundamental research must be done now that the clinical problems have been defined. In the future, the collaboration of molecular biologists and other basic scientists will be needed to illuminate the dark places of our ignorance.

In any textbook, basic scientific knowledge, and the art of clinical practice should be brought together in a rational manner and this volume on cardio vascular therapy attempts to achieve this aim. It deals with five selected areas - hypertension, angina and coronary artery disease, heart failure and anticoa gulant therapy. Clearly not all branches of cardiovascular therapeutics could be included; a separate section on anti-arrhythmic drugs is noticeably absent but it is proposed that this omission will be rectified in other volumes in the series. In general, textbooks on therapeutics tend to be ephemeral; as new discoveries are made and evaluated, medical practice changes. This volume then summarizes current opinion up to mid 1984 and gives, we believe, a reasoned account of present views. The contributors are all clinical pharma cologists with a wealth of clinical experience. The therapeutic advice given is well founded and the underlying scientific basis is clearly explained. The book is aimed at postgraduates, but should the undergraduate care to dip into it, we hope he will be informed and thereby educated. A.
