Pediatric Collections offers what you need to know - original, focused research in a snapshot approach.Although most lesbian, gay, bisexual, transgender, and questioning youths are quite resilient and emerge from adolescence as healthy adults, the effects of stigma and heterosexism can contribute to health disparities. From a public health perspective, transgender and gender diverse (TGD) individuals, compared with their cisgender peers, experience substantially higher lifelong rates of anxiety, depression, self-harm and suicidality, substance use, eating disorders, victimization, homelessness, and incarceration. Pediatric primary care providers (PPCPs) can help foster early access to mental health, family support, and ongoing gender-affirming care that, ultimately, reduces the risk of gender dysphoria, isolation, and shame that many TGD youth unfortunately face. Part 2 of this first-of-its-kind 3-part series can help PPCPs become stronger allies for TGD patients and their families in the clinic, community, and beyond, providing the promise of both a medical home and a future that celebrates people for being true to themselves.

Hone your clinical reasoning and diagnostic skills with a new series of case-based topic-specific studies. This compilation of twenty neurology-related cases includes presenting clinical manifestations and a discussion of the broad range of possible diagnoses, with a reveal of the actual diagnosis at the end. Does this child have a classic presentation of a rare condition or an atypical presentation of a common diagnosis? Does the patient have a primary neurological condition or a neurological manifestation of a systemic disease? Several patients in this collection presented with ataxia, but their final diagnoses range from the unsurprising cerebellar tumor to uncommon conditions like heavy metal poisoning and rare genetic disorders. Does the child with diffuse weakness have a classic condition such as Guillain-BarrÉ syndrome, a vitamin deficiency, one of the periodic paralyses, or an autoimmune disorder such as myasthenia gravis? Cases also include relatively new conditions such as autoimmune encephalitis or genetic disorders but remind us not to overlook well-known but now uncommon conditions like vitamin deficiencies or remote neurological dysfunction due to cancer in another organ.

Pediatric Collections offers what you need to know - original, focused research in a snapshot approach.Digital ecosystems have an outsized impact on children’s development and yet have received considerably less attention than individual behavioral approaches in digital media research (such as a child’s “screen time”). The digital ecosystem includes the digital design patterns that influence children’s media habits by funneling children toward endless scrolls, feeds, and nudges to keep watching. It includes the highly monetized patterns of advertisements and commercialization that are thinly veiled in popular content platforms. It includes the erosion of digital privacy with companies and third parties collecting personal information in covert and overt ways. Part 1 of this series on digital media is an important addition to the literature because it includes studies of how digital design increases children’s risks from media, and how digital media use interacts with larger systemic structures and cultural forces at hand. This important review builds an evidence base justifying the reduction of commercialized content that is so popular in platforms.

Pediatric Collections offers what you need to know - original, focused research in a snapshot approach.Digital media companies are hurtling ahead in a race to capitalize on artificial intelligence (AI) technology, with commercial motives. The evolution of technology continues without enough voices from families and pediatricians to design the technology to suit children’s needs. Indeed, families are adopting technologies at an exponentially more rapid clip with each new technology developed. Part 2 of this book series reflects on previous breakthroughs on the impact of digital media on child and adolescent outcomes, illuminating the ways that digital media can introduce risks through harmful content, displacement of other healthy behaviors, or contact from unwanted adults. This collection delves into how digital media intersects with physical health, such as sedentary behaviors and eating disorders; how digital media intersects with social determinants of health and mental health such as online victimization, problematic internet use, and academic performance. Additional risks identified in this collection explore amplification with artificial intelligence (AI), creating algorithms selecting commercialized content for children with surgical precision, creating sophisticated interactive designs to demand more and more timed play for young children, and widening the systemic and structural disparities that shape children and adolescent’s daily lives and digital experiences.

Pediatric Collections offers what you need to know - original, focused research in a snapshot approach.Technology can be leveraged as a tool to measure digital media use and eventually intervene, including a mobile app to track child smartphone and tablet use, which revealed insights about problematic content often accessed on devices. Future studies could use mobile apps and help families set limits and change design features, to support healthier relationships with media with tailored precision and accuracy. Part 3 of this collection reflects how well-designed technologies including artificial intelligence (AI) have the potential to expand the possibilities of play with imagination and envision a world where child-centered design can improve children’s wellbeing. Digital media can be leveraged as a distraction tool to reduce pain in prehospital settings. As technology becomes more sophisticated and can integrate children’s responses, there is the potential for it to contingently respond to children and act as a social partner. This may provide a more tailored and individualized approach to education and learning that meets children where they are developmentally.

Pediatric Collections offers what you need to know – original, focused research in a snapshot approach. While infections are common, and most are mild and easily diagnosed, the first part of this 2-part series on infectious diseases, "Part 1: Can't Miss Diagnoses," includes cases with diagnoses that may be deadly and are challenging to recognize. Many of the presentations herein are seemingly benign and could easily be dismissed at first glance, and some are initially concerning but are without an obvious diagnosis. However, if these diagnoses are missed, there is a risk of serious morbidity or death and/or significant public health implications. These are the initial presentations that keep us up at night and run through our heads in clinic and at the bedside. They are the diagnoses we don't want to miss. Are you missing something? Are you letting a case report walk out the door? It is our goal that after reading these cases, you will be more alert and open to the idea of unusual and potentially dangerous cases walking into your practice.

Pediatric Collections offers what you need to know – original, focused research in a snapshot approach. The second part of this 2-part series on infectious disease, "Part 2: Indolent Head Scratchers," presents cases of patients not in imminent danger, but sick with puzzling symptoms and histories. Some of these cases have a prolonged clinical course, while others are just plain weird. Some cases present with common findings seen every day in clinical practice, but hide something rare and elusive under the surface, which comes into focus as the case progresses. Some of the diagnoses have been missed or overlooked for quite some time, given the indolent nature of the disease and cognitive biases. Similar to staring at an optical illusion for a long time, the frustratingly elusive diagnosis eventually comes into focus for each case. Together with "Part 1: Can't Miss Diagnoses," this series of infectious disease cases presents serious and puzzling cases designed to help you approach challenging patient cases with an open mind and bring pathogens and disease to light.
