Respiratory failure is a complex disease process whereby the underlying disease and therapeutic measures interact. This book contains an extensive bibliographic review, focusing on preventive and therapeutic studies, that was methodologically standardized, with authors assessing and classifying studies according to statutes of evidence-based medicine. It considers the epidemiology and outcome of mechanical ventilation; addresses ventilator modes and utility of pulmonary mechanics monitoring for treatment; analyzes physiologic effect and patient-outcomes of pulmonary recruitment and lung protective ventilation procedures; describes complications that can be present in these patients such as ventilator-associated pneumonia and useful methods to prevent respiratory infections; covers the impact of bronchodilators, corticosteroids, and antibiotics in acute exacerbation of chronic obstructive pulmonary disease; discusses how, when, and in whom to do tracheostomy; and evaluates the use of sedation and neuromuscular blockade as well as current clinical trials in acute lung injury.

Hemodynamic monitoring is one of the major diagnostic tools available in the acute care setting to diagnose cardiovascular insufficiency and monitor changes over time in response to interventions. However, the rationale and efficacy of hemodynamic monitoring to affect outcome has come into question. We now have increasing evidence that outcome from critical illness can be improved by focused resuscitation based on existing hemodynamic monitoring, whereas non-specific aggressive resuscitation impairs survival. Thus, this book frames hemodynamic monitoring into a functional perspective wherein hemodynamic variables and physiology interact to derive performance and physiological reserve estimates that themselves drive treatment. This philosophy, as well as the limitations and applications of common and evolving hemodynamic measures and their focused use in the care of critically ill patients are discussed, relevant to one underlying truth: No monitoring device, no matter how simple or sophisticated, will improve patient-centered outcomes useless coupled to a treatment which, itself, improves outcome.

There have been tremendous advances in understanding the cellular mechanisms involved in sepsis and contributing to the development of multiple organ dysfunction and mortality in this setting. The chapters in this book provide up-to-date insights into important pathways that are initiated by sepsis.

The coronary circulation is central to sustaining myocardial viability. Unlike the cir­ culations of most other organs, if the coronary circulation becomes insufficient to sustain myocardial contractile function, overall quality of life rapidly declines and life can abruptly end. In partnership with the cerebral circulation, the coronary circula­ tion plays a central role in sustaining life. However, unlike the cerebral circulation, whose function is self-sufficient to define life, the coronary circulation determines global blood flow and thus the initial state for the remainder of the body. This unique condition, together with the reality that coronary artery disease primarily affects peo­ ple in their productive years of life, has allowed physicians and scientists who study the heart to enjoy a privileged position in the field of medical research and clinical practice. Thus, it comes as little surprise that many new and exciting research developments involving the coronary circulation have recently come to the forefront of medical thinking. This reality, coupled with the large number of clinical trials of agents specif­ ically designed to sustain or improve coronary flow in many disease states, makes the timing of this monograph relevant. The book features papers presented at a recent international symposium, the fourth in a series of meetings on Applied Physiology of the Peripheral Circulation. The papers selected by the editors review the most impor­ tant advances in the fields of cardiology and the coronary circulation.

This book brings together basic scientists or clinicians from a variety of different backgrounds - immunology, infectious diseases or critical care - who share a common interest in understanding the changes that occur in immune responses in sepsis. It provides an up-to-date and unrivalled synthesis of current research in this rapidly developing field.

Researchers from many disciplines, with both basic and clinical perspectives, came together in this volume to review and debate issues pertaining to the investigation and control of tissue oxygenation in acute medicine, as well as treatments to improve tissue oxygenation when abnormal. In this latter context, special emphasis is placed on understanding the effect of blood substitutes on the circulation and on the potential roles of this family of compounds in clinical medicine.

Critically ill and critically injured patients require specialized nutrition support to avoid the complications of progressive malnutrition. There is a paucity of information providing practical solutions to these difficult clinical problems. From Nutrition Support to Pharmacologic Nutrition in the ICU focuses on the theoretical and practical aspects of the management of this high-risk patient population. Each chapter presents a state-of-the-art discussion of nutritional and metabolic issues relevant to this resource-intensive patient population and contains current references, liberal tables and figures, and the personal insights of recognized international leaders in this field.

Measuring the quality of a complex service like critical care that combines the highest technology with the most intimate caring is a challenge. Recently, con­ sumers, clinicians, and payers have requested more formal assessments and comparisons of the quality and costs of medical care [2). Donabedian [1) pro­ posed a framework for thinking about the quality of medical care that separates quality into three components: structure, process, and outcome. An instructive analogy for understanding this framework is to imagine a food critic evaluating the quality of a restaurant. The critic might comment on the decoration and lighting ofthe restaurant, how close the tables are to each other, the extent ofthe wine list and where the chef trained. These are all evaluations of the restaurant structure. In addition, the critic might comment on whether the service was courteous and timely - measures of process. Finally, the critic might comment on outcomes like customer satisfaction or food poisoning. Similarly, to a health care critic, structure is the physical and human resources used to deliver medi­ cal care. Processes are the actual treatments offered to patients. Finally,outcomes are what happens to patients, for example, mortality, quality of life,and satisfac­ tion with care (Table 1). There is a debate about which of these measurements is the most important measure of quality.

Cerebral Blood Flow: Quantifying Consciousness Although the heart may be the source of energy needed to generate blood flow,and other organs absolutely essential for normal living,the brain is the reason we are alive. The collected book chapters are aimed at addressing this most fundamental organ and its blood flow. These papers reflect detailed descriptions of similar topics presented over a two-day period as part of the 5th International Symposium on Applied Physiology of the Peripheral Circulation held in Pittsburgh, Pennsylvania, in June 2000. The symposium and this volume were organized into four distinct groupings that follow in a logical fashion. The initial papers focus on the basic unique physiology and pathophysiology of the cerebral circulation, including a historical reviewof the means to measure cerebral blood flow and its implications of the past 30 years. Dr. Walter D. Obrist was one of the original investigators to use radiolabeled markers to assess cerebral blood flow. His equations and initial studies form an impressive introduction to where we are now. As with much of the body's special organs, the brain too has many circulatory features unique to itself.These include the unique blood-brain barrier function ofthe endothelium, local neural regulation control, and intracranial pressure effects. These special issues,plus genetic factors that may predispose individuals to developing cere­ bral aneurysm, make up the initial section of the monograph.

The pathophysiology of sepsis can be regarded as a series of steps, beginning with the invasion of normally sterile tissue by microbes and the elaboration of various pro-inflammatory mediators. The final common pathway is often the development of the multiple organ dysfunction syndrome (MODS). Whereas a great deal has been learned during the past quarter century about the inflammatory processes associated with sepsis (and other related conditions, such as ischemia/reperfusion injury), our understanding is far less developed with respect to the pathophysiological events that lead to organ dysfunction under these conditions. Nevertheless, efforts by both clinical and laboratory scientists are leading to new knowledge in this area. The chapters in this volume provide a state-of-the-art overview of many aspects of the pathophysiology of organ dysfunction in critical illness.

For many years, intensive care has focused on avoiding immediate death from acute, life-threatening conditions. However, there are increasing reports of a number of lingering consequences for those who do indeed survive intensive care. Examples include on-going high risk of death, neurocognitive defects, significant caregiver burden, and continued high healthcare costs.Surviving Intensive Care, written by the world's experts in this area, is dedicated to better understanding the consequences of surviving intensive care and is intended to provide a synopsis of the current knowledge and a stimulus for future research and improved care of the critically ill.
