Kalet, MD, MPH, is a general internist and Professor of Medicine and Medical Education at the Medical College of Wisconsin, Milwaukee, USA.W. Wayne Weston, MD, CCFP, FCFP, is Emeritus Professor of Family Medicine at the Schulich School of Medicine and Dentistry at Western University in Ontario, Canada.
