The stratum corneum is a magnificent example of the successful adaptation of a tissue. Its efficient function is a prerequisite for life itself. We depend on its control of the movement of water through the skin and on its protective role in the prevention of penetration by pathogens or harmful substances. Apart from its functional importance, there is no escaping the fact that the most common skin disorders (psoriasis, eczema, inherited disorders of keratinization, acne) are 'characterised by abnormal keratinization and or scaliness. Despite all this, understanding of its chemistry, structure, physiology and involvement in disease has been sadly deficient; however, there are signs of enlightenment. This symposium is evidence of changed atti· tudes. Although we are still someway off understanding the ultimate causes of such disorders as psoriasis, atopic dermatitis and the congenital disorder of keratinization, we now have considerable information on the physiological disturbances in various diseases. This has permitted and encouraged a rational approach to treatment. The successful use of antimitotic agents, immunomodulators and retinoids may be cited as examples. A major reason for this im provement may be the fact that researchers accept models for the in vestigation of skin diseases. Increasing numbers of them have become available in the past years. So many have been described that it is doubtful whether anyone researcher is aware of all the other models described - even in his own field of interest. This book is a challenge for those involved in the study of skin and its disorders to use the sundry models of skin that have proven helpful. "One Picture Is Worth a Thousand Words" CONFUCIUS The material on which this account is based derives from observations of several thousand of adolescents with acne. All have been treated; many hundreds have been biopsied and have participated in various tests. These constitute our bona fides. Our plan and intentions are as follows: Firstly, we have sought to create a portfolio of still-life pictures of the gross and microscopic anatomy of acne. This will be a photographic record of what acne looks like, of its usual and unusual features, its arch typical as well as of its recondite visages. Secondly, we wish to create a moving picture of acne rather than an atlas. We hope the reader will have the feeling of being in a theater watching an unusual drama. Since anatomy can only come to life when animated by physiology, a vigorous attempt has been made to collate function with structure. It is the eye that observes but it is the mind that sees. Finally, this book is intended for those who wish to understand and treat acne more effectively. The themes are programmed for the clinician. This book will be chiefly a didactic account and not a detailed report of research. We present ourselves as teachers, not as investigators. We conceive it to be the prime mission of medical research to learn how to identify, prevent, and treat the disease. We shall present our personal therapeutic strategies, the ones we use to treat acne patients.
