A variety of authors examine the inner workings of the physician's marriage-the psychological issues and sources of conflict that emerge in the various stages of marriage and family. The authors include notable experts who share their years of clinical experience in helping physicians and their families learn new ways to improve communication, balance the demands of work and family, and grow and change together constructively.

Defined by stable, long-term, subjective distress and/or social impairment, personality disorders affect up to 18% of the population. Social impairment and health care usage are far more prevalent among people with personality disorders than among people with major depressive disorders. Personality disorders are highly prevalent, variable, and notoriously difficult to treat, and they continue to challenge the therapeutic community and represent a formidable public health concern.

This volume ably addresses personality disorders as one of the top priorities of psychiatry for the new millennium, offering a thorough and updated review and analysis of empirical work to point up the issues central to developing a therapeutic model for treatment as well as current research challenges. A review of extant research yields the heartening conclusion that psychotherapy remains an effective treatment for people with personality disorders. An examination of psychodynamic treatment for borderline personality disorder speaks to its efficacy. An analysis of the rationale for combining psychotherapy and psychopharmacology emphasizes the importance of identifying temperament and target conditions. A well-documented and reasoned treatise on antisocial personality disorder makes the crucial point that clinicians must acquire a depth of understanding and skill sufficient to determine what the cut-off point is for treatable versus nontreatable gradations. With the caveat that evidence supporting the efficacy of cognitive treatments for personality disorders is slight and that such approaches require tailoring, a strong case is made for their validity.

This timely volume both answers and reframes many stubborn questions about the efficacy of psychotherapy for treating personality disorders.

This book presents the latest data on-and clinical, ethical, and medicolegal issues pertaining to—sexual intimacy in the professional relationship. Contributors (including psychiatrists, psychologists, social workers, clergy, and attorneys) explore the issue of professional incest across the broad spectrum of the helping professions.

In psychiatry, clinicians recognize the influence of their own personal characteristics on the assessment and treatment of their patients. No interactions in psychiatry are entirely free of countertransference dynamics. Most major theories point to countertransference as a jointly created phenomenon involving feelings induced by the patient combined with the conflicts and preexisting self and object representations that the clinician brings to the encounter.

Countertransference Issues in Psychiatric Treatment provides an overview of theory and technique that gives the reader a detailed account of how countertransference is used in contemporary practice. This timely reference • Illustrates the usefulness of examining countertransference issues in a wide range of psychiatric settings, including pharmacotherapy, consultation-liaison settings, and forensic facilities• Explores the specific countertransference dynamics evoked when clinicians treat suicidal borderline patients and the common countertransference problems confronted when treating antisocial and violent patients• Describes the difficulties encountered in general medical settings when physician-assisted suicide is considered as an acceptable clinical intervention• Breaks new ground in considering psychiatrist's emotional reactions to the patient as an integral part of psychiatric practice and discusses some of the current controversies about countertransference• Reviews cutting-edge psychoanalytic theory involving subjectivity, projective identification, role responsiveness and countertransference enactments

Whereas most literature on countertransference is geared toward psychoanalysis and dynamic psychotherapy, this volume illustrates how countertransference issues must be considered in every clinical setting in which a psychiatrist works. It is an excellent introduction to the topic for psychiatric residents and medical students.

Early in the history of cinema, psychiatrists studied the movies to understand their appeal and power. Meanwhile, filmmakers have long been intrigued by psychiatry and frequently portray this mysterious world in film. Both movies and psychiatry focus on human thought, emotions, behavior, and motivation-making a link between the two subjects inevitable.

Psychiatry and the Cinema explores this complementary relationship from two angles, psychiatrists who have studied the movies and movies that have depicted psychiatry. This second edition looks at: • Over 400 theatrically-released American films that feature psychiatrists or other mental health professionals at work• The stereotypical characters and conventions dominating the presentation of movie psychiatrists and the historical rise and fall of the psychotherapist image in the movies• New trends in psychoanalytically oriented film theory• State-of-the-art understanding of psychoanalytic film criticism and illustrative examples of the use of that methodology with films such as Casablanca, Alien, Three Women, Sea of Love, Working Girl, Good Will Hunting, and many more• Clinical implications of the film representations of psychotherapy for the mental health practitioner

Both entertaining and educational, this book serves as an important aid in understanding the special hold that movies have on audiences.

Because physicians are shaped by the expectations and responsibilities of their profession and are especially susceptible to stress and illness, professionals treating them need to be attuned to a host of demands and considerations not applicable to other patients. The Physician as Patient meets that need by combining the perspectives of two seasoned psychiatrists who have been assessing and treating physicians for more than 30 years. Drs. Myers and Gabbard pool their career-long dedication to physician health, counseling, and risk management to demonstrate that, through accurate diagnosis and state-of-the-art treatment, most impaired physicians can recover and return to practice.

As the most current sourcebook available on this subject, The Physician as Patient not only discusses common illnesses and problems seen in doctors but also outlines the many biopsychosocial treatments that are indicated-always with an emphasis on integrated care. It considers such issues as the reluctance of physicians to assume the role of patient and the uniqueness of the physician's psychological makeup that facilitates or impedes diagnosis and treatment. It describes the most common medical and psychiatric illnesses in physicians—including addictions—and addresses personality disorders and the increasingly important subject of boundary violations. And it helps readers determine what can be done about doctors behaving badly or out-of-character, or what strategies are useful in treating physicians to assure accuracy while also diminishing morbidity and relieving suffering. Among other key issues raised are: • The importance of considering sociocultural customs and values when treating the increasing number of minority and International Medical Graduate physicians • Addressing the many clinical, humanistic, ethical, and legal dimensions of the psychiatric evaluation of physicians• Distinguishing between long-standing patterns of personality disorders that manifest as disruptive behaviors and those patterns that lead to burnout and depression• Assessing amenability to rehabilitation in cases of sexual boundary violation, including an awareness of alcohol or drug abuse as contributing factors• Utilizing a physician's predisposition as a learner and pragmatist to enter into cognitive behavior therapy• The success of early diagnosis and treatment in lowering the occurrence of suicide, for which physicians are notoriously at risk Brimming with case examples, The Physician as Patient adopts a reader-friendly style that facilitates quick grasp of concepts, while an extensive list of references and websites provides an entréeacute;e for additional support. This book is an indispensable resource for all mental health professionals who take caregivers into their care.

Physicians and psychiatrists typically see themselves as true professionals. But in the past, some displayed behavior far beneath the confines of professionalism, including exploding at nurses, not returning calls, or conducting insensitive interactions with patients, that was usually tolerated and seldom disciplined. Today, the rise of professionalism in medicine in general and psychiatry in particular has prompted a quiet revolution in how doctors are trained and how they are expected to behave in the workplace. The Accreditation Council for Graduate Medical Education (ACGME) has now advanced professionalism to be one of the core competencies all emerging practitioners must have.

While almost all physicians believe in professionalism, the movement toward making it a core competency has challenged doctors everywhere to accept the practice of monitoring, observing and assessing what is not always welcome in a field where autonomy is so highly valued. In Professionalism in Psychiatry, the authors identify and expand on professional behaviors, such as being a good team player, being accountable, pursuing improvement in an ongoing way, and behaving compassionately toward patients and families. The importance of treating all co-workers with respect and of being attuned to the management of healthcare resources in a way that reflects fairness and integrity is also thoroughly reviewed. Important features of this book are: • Tailoring professionalism principles from medicine to the unique features of psychiatry in order to enhance educators' teaching and improve the behaviors of psychiatrists and residents in the work setting. • Development of guidelines for professionalism in cyberspace to provide psychiatrists with an ethical framework for dealing with patients in the online realm.• Discussion of the ethical principles that apply when academic departments approach donors.• Focus on cultural competency and empathy in an effort to improve patient care through greater understanding and sensitivity to ethnic, racial, gender and sexual orientation issues encountered in clinical practice. • Use of numerous clinical examples to articulate the new professionalism in psychiatry, which illustrates the importance of going beyond "one size fits all" thinking.

Professionalism in Psychiatry is an important contribution toward beginning to characterize the ever-evolving professional behaviors and clinical strategies of the contemporary psychiatrist and place them in a systematic framework.

All 19 chapters have new references and cutting-edge material that will prepare psychiatrists and residents to treat patients with compassion and skill. The book offers the following features:

• Each chapter integrates new neurobiological findings with psychodynamic understanding so that clinicians can approach their patients with a truly biopsychosocial treatment plan.• Excellent writing and an intuitive structure make complicated psychodynamic concepts easy to understand so that readers can grasp the practical application of theory in everyday practice.• The book links clinical understanding to the new DSM-5 nomenclature so that clinicians and trainees can adapt psychodynamic thinking to the new conceptual models of disorders.• New coverage of psychodynamic thinking with relation to the treatment of patients on the autism spectrum addresses an increasingly important practice area.• Posttraumatic stress and dissociative disorders have been combined to allow for integrated coverage of primary psychiatric disorders related to trauma and stressors.

A boon to clinicians in training and practice, the book has been meticulously edited and grounded in the latest research. The author firmly believes that clinicians must not lose the complexities of the person in the process of helping the patient. Psychodynamic Psychiatry in Clinical Practice, Fifth Edition, keeps this approach front and center as it engages, instructs, and exhorts the reader in the thoughtful, humane practice of psychodynamic psychiatry.

The definitive treatment textbook in psychiatry, this fifth edition of Gabbard''s Treatments of Psychiatric Disorders has been thoroughly restructured to reflect the new DSM-5® categories, preserving its value as a state-of-the-art resource and increasing its utility in the field. The editors have produced a volume that is both comprehensive and concise, meeting the needs of clinicians who prefer a single, user-friendly volume.
