The past decade has been highlighted by numerous advances in research of medical scientific knowledge, medical technology and the biological and diagnostic techniques-but somewhat less dramatic changes or improvement in management of medical conditions. This volume in the Emerging Infectious Disease of the 21st century series, addresses some of the emerging issues and controversies in Infectious Diseases.

The author has chosen these topics for review based on questions and contentions raised by medical residents, internists, emergency physicians, clinical infectious disease specialists and family physicians. This volume will provide guidance and answers to frequently asked questions in infectious diseases to the researcher, clinician, generalists, specialists and trainees in these areas; thus, facilitating improved patient care, prudent and cost effective management and investigation of these disorders.

This volume reviews the research, diagnosis, and treatment of some common infections facing researchers, clinicians and family physicians such as sinusitis, otitis media and asymptomatic bacteriuria. Recent studies and surveys have shown that these conditions are often over diagnosed and treated unnecessarily with antibiotics. The approach and guidelines for diagnosis and management are reviewed in this volume. Other more complicated but less common conditions challenging internists, clinical infectious disease consultants and other specialists are also reviewed (i.e. meningitis, ventilator associated pneumonia, sepsis, hepatitis C, B etc.)

Antimicrobial resistance can develop in any type of microbe (germ). Microbes can develop resistance to specific medicines. A common misconception is that a person’s body becomes resistant to specific drugs. However, it is microbes, not people that become resistant to the drugs. Drug resistance happens when microbes develop ways to survive the use of medicines meant to kill or weaken them. If a microbe is resistant to many drugs, treating the infections it causes can become difficult or even impossible. Someone with an infection that is resistant to a certain medicine can pass that resistant infection to another person. In this way, a hard-to-treat illness can be spread from person to person. In some cases, the illness can lead to serious disability or even death.

This comprehensive, up-to-date volume aims to define issues and potential solutions to the challenges of antimicrobial resistance. The chapter authors are leading international experts on antimicrobial resistance among a variety of bacteria (Streptococcus pneumoniae, enteroccoci, staphylococci, gram-negative bacilli, mycobacteria species) viruses (HIV, herpesviruses),  and fungi (Candida species, fusarium etc.). The chapters will explore the molecular mechanisms of drug resistance, the immunology and epidemiology of resistance strains, clinical implications and implications on research and lack thereof, and prevention and future directions. This volume will also describe the steps that researchers are taking to develop molecular methods for detecting resistance; develop drugs and other means to deal with newly-resistant organisms. A special chapter to address the issues on strategies to limit antimicrobial resistance propagation will be included in this volume.

This new volume in the series Emerging Infectious Diseases of the 21st Century is a novel book on the role of microbes in the pathogenesis of common and disabling non-infectious diseases. New insights have emerged over the past several years suggesting that our commensal microflora of the gut is extremely important in regulating physiological and immune functions of the body. Covered are the perturbations of the normal composition of our endogenous microbiota, influenced by diet and genetic predispositions, as well as the mechanisms to produce common disorders such as obesity, diabetes, irritable bowel syndrome, colon cancer and atherosclerotic vascular diseases.
