A Companion to the Classification of Mental Disorders provides essential reading as a background and supplement to both the recently produced DSM-5 and the forthcoming ICD-11. It focuses on the processes of classification and diagnosis, and the uses for these classifications. The book emphasises the dangers of regarding any current psychiatric classification as true or complete, in view of the present partial state of knowledge about the causes and mechanisms of most mental and behavioural disorders. This book is unique in containing a number of chapters that give a brief history of the cooperative efforts and projects necessary for the production of internationally agreed psychiatric classifications. The discussion begins in 1965 with the US/UK Diagnostic study, the preparations for ICD-8 with its first international glossary, and the International Pilot Study of Schizophrenia, designed and coordinated by the World Health Organization. While recognizing the importance of the innovations of the DSM series of classifications of the American Psychiatric Association, the book also takes a truly international perspective. The expert authors are well placed to do this, having been personally involved in many of the collaborative studies and developments discussed. A Companion to the Classification of Mental Disorders is an illustration of how much international collaborative work has been necessary over several decades to produce the currently agreed classifications. There is still a long way to go, but a start has been made.

This book is a report of the findings of the International Study of Schizophrenia (ISoS), Focusing on variations in the course and outcome of schizophrenic disorders, the investigation covers 14 countries in both the developed and developing world. The bulk of the volume consists of portraits of individual field research centres in each country and reports on the outcomes of these centres' schizophrenic patients. The "portrait chapters" are flanked by introductory and synoptic chapters laying out both the genealogy and design of ISoS and synthesising its major findings. Of these, the most significant conclusions: that while recovery from schizophrenia is a struggle for many patients, it is possible to achieve in terms of improved daily function and quality of life and that, with appropriate and sustained treatment, schizophrenia is largely an episodic disorder that has a favourable outcome for a significant portion of those afflicted with it. The book also includes extensive tables that present the research data, permitting further independent analysis.Recovery from Schizophrenia is unique - there is virtually nothing like it in the contemporary field of cross-cultural psychiatric epidemiology, and the massive, multinational investigations upon which it is based are not likely to be replicated any time soon in the area of mental illness. As such, this book will be a unique resource for mental health professionals, practitioners, and researchers worldwide, providing an empirically based reason for hope in the long run for persons living with schizophrenia.

In 2012, Paradigms Lost critically examined the key paradigms used in programs aiming to prevent or reduce the stigma attached to mental illness. Today, many programs addressing stigma have been successfully developed or improved using the notions and recommendations presented in that volume. Paradigms Lost, Paradigms Found builds on the lessons of the first edition and adds new approaches to reducing the stigma related to mental illness. It brings together the latest theory and experience in the field to provide effective recommendations for addressing stigma in its various forms. This second edition also presents targeted programs used by different social groups and explores future directions in stigma prevention, including evolving techniques for the digital age. Integrating experience with modern technology, Paradigms Lost, Paradigms Found provides evidence and inspiration for mental health advocates seeking to prevent or reduce stigma of mental illness and protect the rights of people with mental and substance use disorders.

Paradigms Lost challenges key paradigms currently held about the prevention or reduction of stigma attached to mental illness using evidence and the experience the authors gathered during the many years of their work in this field. Each chapter examines one currently held paradigm and presents reasons why it should be replaced with a new perspective. The book argues for enlightened opportunism (using every opportunity to fight stigma), rather than more time consuming planning, and emphasizes that the best way to approach anti-stigma work is to select targets jointly with those who are most concerned. The most radical change of paradigms concerns the evaluation of outcome for anti-stigma activities. Previously, changes in stigmatizing attitudes were used as the best indicator of success. Paradigms Lost and its authors argue that it is now necessary to measure changes in behaviors (both from the perspective of those stigmatized and those who stigmatize) to obtain a more valid measure of a program's success. Other myths to be challenged: providing knowledge about mental illness will reduce stigma; community care will de-stigmatize mental illness and psychiatry; people with a mental illness are less discriminated against in developing countries. Paradigms Lost concludes by describing key elements in successful anti stigma work including the recommended duration of anti-stigma programmes, the involvement of those with mental illness in designing programmes, and the definition of programmes in accordance with local circumstances. A summary of weaknesses of currently held paradigms and corresponding lists of best practice principles to guide future anti-stigma action and research bring this insightful volume to an apt conclusion.

"Adapt or perish, now as ever, is Nature's inexorable imperative. " -H. G. Wells, Mind at the End of Its Tether (1946) Doctors are trained to treat people suffering from various diseases. This is the main form of their activity and usually the reason for which they selected medicine as their profession. The notion that they should become managers and engage in activi ties such as programming, calculating cost, assessing cost-benefit ratios, and thinking about pricing in accordance with the social utility of their intervention, is both foreign and abhorrent to them. They are sometimes willing to say how much they need in order to have a well-functioning service: usually they prefer to state what specific apparatus and other things they require without specifying the price of their demand. They can be persuaded to add a price tag to what they think is necessary for their work: but that was about as far as they would go, until recently. The growing emphasis on human rights over the past few decades, the greater emphasis on quality of life and the public's heightened expectations about their health led, in many industrialized countries, to a greater demand for health services. This, com bined with improved possibilities of diagnosis and treatment (at higher cost!), led to a significant increase in financial demands which made governments and health-care systems uneasy and ready to accept any solution that would stop the spiral of seem ingly endless cost augmentation.

Research evidence concerning the precursors (a sign or symptom that heralds another) and prodromes (an early or premonitory symptom of a disease) of the various mental disorders is growing but is at present widely dispersed and inaccessible. Clinicians have to rely on their own experience, which is often limited, when faced with several delicate diagnostic and treatment issues. This book aims to approach for the first time in a comprehensive way this emerging area of clinical psychiatry. The World Health Organisation has found that mental disorders such as depression and psychosis rank in the top 10 of leading causes of disability in the world, creating a significant social, emotional and economic burden for young people, their families and society. Early detection of these potentially disabling disorders and treatment at the time of initial onset can greatly improve long-term outcomes and effect dramatic cost-savings. The related Birchwood title has sold nearly 2300 copies and is popular at psychiatry conferences. From an R&D perspective, industry have a stake in this area. Obviously, if biomarkers or other prodromal evidence can be determined in various disorders, then drugs will start to be dispensed earlier. The topic has increased in popularity as a subject for satellite symposia e.g Jeffrey Lieberman chaired the Bristol Myers Squibb symposia at the American Psychiatric Association. This dealt with early intervention in schizophrenia and the need for novel approaches in the prodromal phase. In addition, more symposia (and research) look at disorders "across the lifespan." This phrase indicates an approach to research, diagnosis and treatment from a more quality-of-life vantage and early diagnosis is part of this movement.

According to the World Health Organization’s World Health Report 2001, one family in four worldwide has at least one member currently suffering from a mental disorder. This disorder often remains undiagnosed and untreated, but the behaviour of the ill person usually has a significant impact on the quality of life of the family and on the mental health of the other members of the family, and generates feelings of shame, guilt, helplessness and despair.

This book provides a comprehensive picture of currently available evidence about the specific characteristics of the burden on the families of people with the various mental disorders, the coping strategies which increase or decrease this burden, the family interventions of proven efficacy, and what should be said and what should not be said to the relatives of people with the various conditions.

Around the world, a vast number of children and adolescents suffer from mental and neurological disorders and only a small proportion of them receive adequate care. This is so in both developing and developed countries. The need to raise awareness about this problem and provide specific advice regarding their prevention and treatment was identified as a priority for the World Psychiatric Association by Professor Ahmed Okasha during his Presidency of the Association and resulted in the creation of his Presidential Programme on Child Mental Health. This book presents some of the fruits of this programme and constitutes a global call to action for mental health workers and policy makers.

The Mental Health of Children and Adolescents: An area of global neglect brings together information on the burden of mental disorders in childhood and adolescence with methods of raising public awareness of these problems. The book also describes therapeutic and preventive interventions that could diminish them. It then presents the results of field studies in Brazil, Egypt and Russia documenting the effectiveness of interventions to prevent school dropout in those three very different countries.

With its emphasis on practical guidance for the development of interventions to prevent or treat child and adolescent mental health, The Mental Health of Children and Adolescents: An area of global neglect provides much-needed information of direct interest to health professionals and the many others involved in the provision of health care to children and adolescents in both the developing and the developed countries.

In recent years, there has been a growing awareness of the multiple interrelationships between depression and various physical diseases. The WPA is providing an update of currently available evidence on these interrelationships by the publication of three books, dealing with the comorbidity of depression with diabetes, heart disease and cancer.

Depression is a frequent and serious comorbid condition in diabetes, which adversely affects quality of life and the long-term prognosis. Co-occurrent depression presents peculiar clinical challenges, making both conditions harder to manage.

Depression and Diabetes is the first book devoted to the interaction between these common disorders. World leaders in diabetes, depression and public health synthesize current evidence, including some previously unpublished data, in a concise, easy-to-read format. They provide an overview of the epidemiology, pathogenesis, medical costs, management, and public health and cultural implications of the comorbidity between depression and diabetes. The book describes how the negative consequences of depression in diabetes could be avoided, given that effective depression treatments for diabetic patients are available.

Its practical approach makes the book ideal for all those involved in the management of these patients: psychiatrists, psychologists, diabetologists, general practitioners, diabetes specialist nurses and mental health nurses.

Depressive disorders have profound social and economic consequences, owing to the suffering and disability they cause. They often occur together with somatic illness which worsens the prognosis of both. Prevention, detection and optimal treatment of these disorders are therefore of great clinical and economic importance.

This edition of the first title in the acclaimed Evidence & Experience series from the World Psychiatric Association has been fully revised and features a new section on depression in primary care – the main channel for the management of these disorders in countries around the world.

The format remains a systematic review of each topic, evaluating published evidence, complemented by up to six commentaries in which experts provide valuable insight gained from clinical experience.

All the evidence, systematically reviewed and analysed, in one place.  Practical context imparted in expert commentaries from around the world, which were highly popular in the previous edition. Provides an unbiased and reliable reference source for practising psychiatrists and physicians everywhere. Features a new section on the treatment of depression in primary care. Edited by a highly experienced, internationally renowned team.

This book will be informative and stimulating reading for everyone working with people with depressive disorders in all countries and settings: psychiatrists, psychologists, primary care physicians and other mental healthcare professionals.

Review of the first edition

“The discussion papers are excellent.  I strongly recommend this masterfully edited book, which remarkably succeeds in combining research evidence and clinical experience.  It is probably the most helpful update on depression available today, both for the researcher in mood disorders and the practising clinician.” S. Grandi in Psychotherapy and Psychosomatics, 2000

Bipolar disorder is a serious mental disorder involving episodes of serious mania and depression and affects approximately one to three percent of the population. According to the National Institute of Mental Health nearly two million individuals in the United States alone are diagnosed with this disorder. * This title aims to provide an overview of recent research progress * It explores the impact of this evidence on the practice of expert clinicians of many different countries * It will be an unbiased and reliable reference point with the kudos of WPA endorsement

Dementia is most frequently associated with aging and is, at present, under-diagnosed and under-represented all over the world. Discrepancies in the diagnostic procedures and therapeutic interventions implemented in the various clinical contexts are significant and consequently, the need for a review of the currently available research evidence and a discussion of different clinical practices is urgently needed. This book provides a much needed review of the diagnostic procedures and the implementations of therapeutic interventions.

Provides accompanying commentaries by an outstanding line up of contributors Covers developments in diagnosis, therapy, prognosis, economic evaluation and quality improvement Provides an unbiased and reliable reference point

Obsessive-compulsive disorder is probably the psychiatric disorder for which most significant progress has been made on the last 20 years concerning pharmacologic and psychotherapeutic interventions. A number of studies have shown that OCD is much more prevalent than previously thought, occurring in an estimated 2% of the adult population around the world. A serious discrepancy still exists between research evidence and clinical practice and an update of this evidence and an international debate on it, as provided by this volume, is long overdue. This revised edition provides vital information on a considerably underdiagnosed condition. * Provides accompanying commentaries by an outstanding line up of contributors * Covers developments in diagnosis, therapy, prognosis, economic evaluation and quality improvement * Provides an unbiased and reliable reference point

From Chapter 1 "Depression, like many other mental disorders, is characterised by the presence of a number of symptoms which are changeable over time." Among these symptoms are a depressed mood and a loss of interest; physical and cognitive changes may also occur. Because of difficulties in diagnosis (and varied criteria) only estimates in the global number of cases of depression can be made. Some estimates put it in front of chronic heart disease as a health problem and cause of death. This revised edition covers developments in diagnosis, theraphy, prognosis, economic evaluation and quality improvement. * Provides accompanying commentaries by an outstanding line up of contributors * Covers developments in diagnosis, therapy, prognosis, economic evaluation and quality improvement * Provides an unbiased and reliable reference point

Recently, there has been a growing awareness of the multiple interrelationships between depression and various physical diseases. Patients with psychiatric problems, particularly depression, may be more susceptible to cardiovascular disorders. Depression and Heart Disease synthesizes current evidence, including some previously unpublished data, in a concise, easy-to-read format. The authors succinctly describe the epidemiology, pathogenesis (including cytokines and genetics), and risk factors of the comorbidity between depression and heart disease. The book also reviews the best pharmacological and psychotherapeutic approaches for people with this comorbidity.

Recently, there has been a growing awareness of the multiple interrelationships between depression and cancer. Depression and Cancer is devoted to the interaction between these disorders. The book examines various aspects of this comorbidity and describes how the negative consequences of depression in cancer could be avoided or ameliorated, given that effective depression treatments for cancer patients are available. Renowned psychiatrists and oncologists summarize the latest evidence on the epidemiology, pathogenesis, screening and recognition, and cultural and public health implications of depression in persons with cancer, among other topics.
