The notion of a single political organization encompassing the whole of humanity—a world state—has intrigued mankind since earliest recorded history. This book provides a concise yet comprehensive overview of the history of world government, and questions whether political globalization, in the form of a federal world government, could and should complement the ongoing processes of economic and cultural globalization. While the potential peacekeeping advantage of such a state is obvious, the consensus judgment has always been against it, because it could lead to totalitarian tyranny. Yunker examines whether this judgment is still correct, considering that nuclear weapons of unimaginable destructiveness now exist, capable of destroying human civilization as we know it. Summarizing the lessons of history, the author suggests that while the conventional world federalist concept of an unlimited world government is still impractical in today’s world, there may be a role for a limited federal world government that would go well beyond the existing United Nations, thereby providing a stronger institutional basis for the evolutionary development of genuinely effective global governance. This book is an important resource for all students and scholars of global governance, international relations and international organizations.

The later stages of dementia are as important, if not more so, as the earlier stages, since they harbour unique characteristics and events, which profoundly affect the lives of patients and their carers. Severe dementia has not had a high profile in the clinical literature as until recently prognosis was poor and there were few beneficial interventions. With the recent licensing of memantine, clinicians finally have a drug option that will delay disease progression. Severe Dementia is the first book to focus exclusively on severe dementia. It addresses both the clinical features of the disease and the social aspects of care. Introductory chapters on the differential diagnosis, neurochemistry and molecular pathology of severe dementia set the scene for the clinical discussion. Detailed clinical chapters on cognitive function, depression, physical effects, staging and function follow. All therapeutic interventions are then discussed, including memantine, anticholinesterases, neuroleptics and non-pharmacological treatment. The final chapters review the social and economic aspects of dementia care, including family involvement, person-centered care, palliative care, ethics and health economics.Written and edited by experts in geriatric psychiatry and geriatrics, Severe Dementia is of value to all clinicians involved in the management of this complex and vulnerable group of patients. It is also of interest to general practitioners and carers in nursing homes.

Depression is one of the most common mental health problems facing older people, and it is often unrecognised and usually under-treated. Integrated Management of Depression in the Elderly provides an entirely new approach to understanding late-life depression, by using a series of case studies with commentaries from practitioners internationally. The book covers the epidemiology, presentation and diagnosis of depression in older people and outlines current evidence for effective management drawing on recently published work. The substantive part of the book presents ten case studies of increasing complexity, each case has a commentary from a primary care clinician and a health or social care professional, to outline how professionals should work together to manage the patient within their community. Contributions from world experts give the book an international appeal. It will appeal to a wide audience of health and social care professionals together with psychiatrists-in-training.

This unique evaluation of the outcomes of residential and nursing home care for older people identifies the factors determining the quality of life of older people who have moved into care homes. It examines the relationship between older people's psychological well-being and the kinds of care received in residential homes. The volume draws on a study of UK care homes, interviewing new entrants soon after admission and then on two further occasions, to ascertain their experience of care and their quality of life. Interviews were also undertaken with care staff and their managers, and the care environment of each home was assessed. The authors provide valuable evidence of the factors which can influence older people's well-being on entering a care home and how they adjust either positively or not to their new surroundings. The volume offers clear pointers towards ways to improve quality of residential and nursing home care.
