Health systems in low- and middle-income countries: an economic and policy perspective outlines the key aspects and issues concerning health systems of low- and middle-income countries, recognising the current global context within which these systems operate and the dynamics of this context. It brings together a set of renowned authors to tackle the issues that face population health and health care in the twenty-first century. The focus is predominantly low- and middle-income countries, with a distinct meeting of economic and policy perspectives, and grounding analysis of key issues within the broader international context. The book therefore provides a unique and comprehensive analysis of health systems, with a very different and unique 'flavour' in the field. It should have wide appeal to those engaged in health and health care, including health professionals, researchers and students, as well as those outside traditional health professions but with increasing interest within health issues, such as trade policy makers, researchers and students.

Drawing on the traditional "hillbilly" music of his Kentucky childhood, Bill Monroe created the "high lonesome sound" of bluegrass; his accomplished mandolin-playing and haunting high-range voice, backed by fiddle and five-string banjo. The youngest child of eight, Monroe lost both his parents while still a teenager, and, as an adult, was a guarded, extremely private man. He was also a compulsive womanizer, whose longtime affair with the wife of a Tennessee highway patrolman was an open secret in Nashville. As one of the early stars of the "Grand Ole Opry" he inspired many young stars including Johnny Cash and Elvis Presley (whose first single for Sun Records was Monroe's "Blue Moon of Kentucky"). Ironically, the success of rock 'n roll caused Monroe some lean years in the 1950s but by the early 1960s, the folk revival movement had rediscovered him as "the Father of Bluegrass" and he found a whole new audience which continues today. His accomplishments are ranked with those of other musical giants including Duke Ellington, Louis Armstrong, Charles Ives and Hank Williams. He died in 1996, aged 84.

The arguments and violent tempers within their group, the violence they are subjected to by others, and the insecurity they must live with after the massacre of the 7th Cavalry.

This is a story set in the 1870s of violence and struggle in the West and of a mother and her daughter who find themselves cast into slavery to settle a debt not of their doing. This is a story of a mother forced to submit to the vilest of tasks in the belief that it will protect her daughter from the same plight and her daughter who will in time learn what her mother is being forced to do and she will soon be forced to do the same. This is a story of a mother and her daughter whose belief in God helped them keep their minds and souls above their truly hell-like world they find themselves in and the heartbreak which awaits them.
