Throughout the world, healthcare professionals often lack knowledge of the possibilities and limitations of systematically processing data, information and knowledge and of the resulting impact on quality decision-making. They are often asked to use information technologies of which they have limited appreciation, in order to enhance their practices through better use of information resources. However, for systematically processing data, information and knowledge in medicine and in healthcare, healthcare professionals who are well-trained in medical informatics or health informatics are needed. It will only be through improved education of healthcare professionals and through an increase in the number of well-trained workers in health and medical informatics that this lack of knowledge and associated skills can begin to be reversed. The topics of Advances in Information Technology and Communication in Health, the proceedings of ITCH 2009, include telemedicine and telehealth, electronic health records, software assurance and usability, terminology, classification and standards, software selection and evaluation, research and development initiatives, service administration, management and self-management, nation-wide Canadian initiatives, ethics, policy and government, decision support, artificial intelligence and modeling, software design and development, educational initiatives and professional development and technology adoption and evaluation. In March 1986, a Canadian colloquium with an international flavor was convened to discuss the impact of information technology on community health. It was sponsored by the School of Health Information Science at the University of Victoria and the British Columbia Ministry of Health. This small, successful gathering was the predecessor of the Information Technology in Community Health (ITCH) conferences that followed in 1987, 1988, 1990, 1992, 1994, 1996, 1998 and 2000. In 2007, after a brief hiatus, the conference was held again, but this time it had expanded its scope. It was known as Information Technology and Communications in Health (ITCH) 2007; with the same acronym but with a different meaning as demanded by its international appeal and wider choice of subject areas. The conference in 2007 was an unmatched success and for the conference of 2009, an even more eventful convention is expected, which encourages experts to demonstrate and share their experiences and knowledge. The theme for the ITCH 2009 conference is ‘Revolutionizing Health Care with Informatics: From Research to Practice’.

The accession of new EU member states demands considerable effort on their part, aimed at developing common policies and strategies. Over the last two years we have tried to encourage the community of medical informatics professionals and researchers from Central and Eastern European countries to undertake these challenges and join European activities in the field of e-health. We are aware of the fact that only common standards and procedures will enable full integration of this region with the main stream of Information Society developments, currently accelerating all over Europe. This book also reports on the results of cooperation between researchers and centres representing both new EU entrants and those countries, that have been part of the Community for a long time. The European Commission has consistently fostered the strategy of e-health development over the past decades. Yet, the economic transformation underway in Central and Eastern European countries does not favour rapid development in this domain. Healthcare systems all over the region are being transformed in search of more effective mechanisms of financing. One of the main motivations for publishing this book is to exchange ideas and share experience which can speed up e-health services development in Central and Eastern countries. The development and advancement of personalised health management systems requires the consideration of advances in sensor technologies and advanced textiles in addition to nano technologies and evolving information and communication technologies. Today, biologists and medicinal chemists realize that there is a strong relationship between pharmacodynamic (what the drug does to the organism) and pharmacokinetic (what the organism does to the drug) effects. A significant contributing factor to the evolution in drug discovery was the methodological and technological revolution with the advent of combinatorial chemistry, high-throughput screening and profiling, and in silico prediction of target-based activity and ADMET (absorption, distribution, metabolism, excretion and toxicity) properties. High-throughput screening and in silico methods have accelerated the process towards drugability of new chemical structures. Another component of the revolution in drug discovery is the replacement of the disease (indication)-based approach by a target-based approach. A better understanding of pathophysiology of diseases and the underlying biological processes of diseases combined with explosive development of genomics and proteomics have been instrumental in the birth of this new paradigm. This volume summarizes discussions of these three aspects of modern drug discovery, i.e. priority for targets, early ADMET assessment, and in silico screening. We trust that readers from academia as well as from industry will benefit from these studies.

We humans are tribal, grouping ourselves by a multitude of criteria: physical, intellectual, political, emotional, etc. The Internet and its auxiliary technologies have enabled a novel dimension in tribal behavior during our recent past. While the benefits of innovation trickle downward, from the privileged few to the broader masses, we should expand this trickle into a flood. Can breakthrough applications in stimulation, visualization, robotics, and informatics engender tools as ingeniously as the water carrier or laptop? The year 2008 could be viewed as a time of potentially exciting breakthroughs in our understanding of the deformity of scoliosis. The rapid advances in imaging technology will allow better and more detailed images of the spine, both on its surface and deeper internally, using techniques such as Laser scanning, Magnetic Resonance Imaging and ultrasound. There is little doubt these technologies are going to advance massively, new ones will come into the hands of clinicians and researchers and a better understanding of the complex functional anatomy of the spine will be developed. This will undoubtedly aid biomechanicians to model the spine and its function, under gravity and movement, allowing new insight into progressions of curves and ways to surgically control deformities. However, it is probably the potential of biology and medical research which offers the greatest opportunities to further our understanding. The incredible advances at the molecular level, the expanding knowledge of genetics and the recent discoveries in the field of neurophysiology offer -for the first time- real potential for unraveling the puzzles of etiology. To find a solution, we must build on these new developments and look beyond the spine to the whole body to discover if its biological functions are disturbed. As the United States continues to debate reform of its healthcare system, this book argues that providing health insurance for all without improving the delivery system will not improve the current problems of access, affordability, and quality. Decisions regarding the risks involved in medical treatments must belong to patients and their physicians – after all, it is the patient's health and life which is at stake. But patients will not be equipped for this decision-making process if they cannot be given some idea as to the risks and benefits of treatment. Such risks are generally estimated by a consensus panel of specialist physicians using supporting medical literature. Unfortunately, this literature does not always provide a good estimate of risk, particularly in the case of rare occurrences. This book demonstrates statistical techniques that can be used to investigate matters of risk. These include kernel density estimation, predictive modeling, association rules and text analysis. It also shows, through example, how these techniques can provide meaningful results, and examines current methods, discussing some of the flaws in models which may lead to misleading results. After a general introduction to the concept of medical risk, the subjects covered include the process by which rare occurrences are investigated in drugs or treatments, the trade-offs between risks and benefits, extrapolation of clinical trial results and the cost of healthcare in relation to risks.
