The book describes key socio-political reforms that helped shape post-apartheid South Africa’s mental health system. The author interrogates how reforms shaped public, community-based services for people living with severe mental illness, and how features of this care has been determined, in part at least, by the relations between actors and structures in the state, private for-profit health care, and civil society spheres. A description of the development of South Africa’s post-apartheid health system, and the contentions that emerge therein, sets the stage for an analysis of the country’s most tragic human rights failure during its democratic period, namely the Life Esidimeni tragedy. The roots of the tragedy are not only framed as a loss of life and dignity as a result of political corruption and administrative mismanagement, but as a power differential that ultimately highlights an unjust system that relegates its most vulnerable citizens to commodities, without voice and without agency. The book concludes that the commodification of severe mental illness has been a product of neoliberal discourses that have shaped the economistic ways in which the post-apartheid South African state have governed poverty and severe mental illness.This book will be of interest to scholars of health, social and economic policy in South Africa.

This book explores the normalization of HIV and AIDS, reflecting upon the intended and unintended consequences of the multifarious "AIDS industry."The Normalization of the HIV and AIDS Epidemic in South Africa deals with the manner in which the HIV and AIDS epidemic has become such a well-known disease with such wide-ranging ramifications. This book provides an overview of the current epidemiology of the HIV epidemic among young people in Eastern and Southern Africa (ESA) and examines the efforts to confront and reduce the high level of new HIV infections amongst young people.Taking a multi-dimensional approach to prevention, the contributors discuss the many challenges facing these efforts, in view of the slow progress in curbing the incidence of HIV amongst young people, focusing particularly on the structural and social drivers of HIV. This book investigates how mental health in South Africa is conceptualised and constructed in public policy. Critiquing embedded assumptions within existing policy documentation, the book advocates for policy solutions centred on poverty alleviation and economic development.Mental health in South Africa has historically been neglected within the health-care system, a stark reality underscored by the Life Esidimeni tragedy, which exposed widespread mismanagement, negligence, and insufficient resources in mental health-care services. Over the past 30 years, an ever-expanding community of global health actors have been intervening in the poorest parts of the world with the aim of combatting disease and improving health outcomes. While many global health programmes in the Global South have been successful in saving lives, their successes have often been compromised by poor coordination, inefficiency and a lack of alignment with national priorities.This book outlines the deep historical and political dimensions of this fragmentation, using rural Zambia as a case study. In the name of global health, different actors produce and circulate radically different stories about how to improve health. These narratives not only contribute to a fragmented landscape of health provision but also create more subtle and pervasive forms of fragmentation in the everyday lives of health workers, patients and government officials. Based on long-term ethnographic fieldwork in rural Zambia, this book examines three different global health interventions driven by differing stories of salvation, community and experimentation. Tracing the longer history of these stories, the book shows how they have failed to account for the realities of life in rural Zambia.Providing an innovative and nuanced analysis of global health that challenges the idea of fragmentation as simply a technical problem, this book will be an important read for researchers of global health and African studies.
