This atlas was written for general pathologists, dermatopathologists, and pathologists in training with an interest in practical diagnostic issues in the pathology of melanocytic tumors. The focus of this fascicle is on microscopic clues for diagnosis, in particular for the distinction of melanocytic nevi from melanoma. The authors also discuss the role of ancillary methods and their potential value as well as limitations in test sensitivity and specificity. This volume will help pathologists provide the most accurate diagnoses in service of their clinical care teams and their patients.

It is focused on diagnostic histopathology, discussing and lavishly illustrating disease patterns with an emphasis on differential diagnosis. The key aspects of clinical features, laboratory findings, and pathophysiology are woven into this book. The application and interpretation of special stains and immunohistochemistry are integrated into discussions of each disease pattern.

This volume builds upon the work of its predecessor by liberally illustrating the spectrum of each tumour and disease described, whilst providing many new illustrations. Morphometry, flow cytometry, immunohistochemistry, gene rearrangement studies, and other molecular biologic methodologies are exciting new areas that are being developed into diagnostic procedures in the pathology laboratory. Where these methodologies are applicable to salivary gland disease, they are included. Histomorphologic evaluation, however, remains the principal and most useful method for diagnosis of salivary gland tumors and is the emphasis of this edition. A differential diagnosis is included in the discussion of most tumours, and the cytopathologic features of those tumours most commonly encountered in the cytopathology laboratory are discussed.

Offers a concise guide to understanding the placent - a unique organ involving both mother and child during pregnancy. Divided into fourteen chapters and an appendix and with 490 illustrations, it includes the structure and function of the normal placenta and then proceeds to review the major disorders and lesions, which occur as complications of a normal pregnancy and delivery. It includes not only pathology authors but also an obstetrician and radiologist who present an expert comprehensive review of diseases of the placenta. It is therefore a valuable reference for all three specialty groups in this field.

The twenty-six chapters in this Atlas are organised around standardised disease entities or patterns of disease, including congenital and hereditary diseases, acquired diseases of the native kidney, and diseases of the renal allograft. Wherever possible, the authors have tried to remain consistent with the widely-recognised International Nomenclature of Disease, a joint project of the Council for International Organizations of Medical Sciences and the World Health Organization.Although this Atlas provides comprehensive coverage of the major common disease entities and their variants, it also includes many rare, but no less important, conditions. The extensive collection of images is the focal point, balanced by an informative text and selective use of illustrative tables.

The pathologist who sees specimens from lesions of the serosal membranes (pleura, pericardium, peritoneum, and tunica vaginalis) is well aware that these cases, whether in the form of fluid cytologies, small biopsies, or major resections, pose differential diagnostic problems that are among the most difficult in anatomic pathology. The main problems involve the separation of malignant mesothelioma from benign mesothelial processes on the one hand and other malignant tumours (particularly primary and metastatic carcinomas) on the other. With these differential diagnoses in mind, the authors concentrate on the cytopathologic and histopathologic features of diffuse malignant mesothelioma, its many variants, and the other benign and malignant lesions to be distinguished from it. Histochemical, immunohistochemical, ultrastructural and molecular features of potential use in this differentiation are discussed and illustrated. Common differential diagnostic problems and their solutions take precedence, but all of the known benign and malignant lesions of serosal membranes are covered, with copious illustrations and the latest available references. Epidemiologic, pathogenetic, clinical, radiographic and gross pathologic features are integrated with the discussions of these entities. This will be the most complete source available on this topic for years to come.

One of the main reasons that this edition was undertaken is that for some years there has been no single, up-to-date book containing extensive information on benign and reactive lymphoid conditions. Although malignant processes garner the majority of attention, it is the benign processes that have an underlying complexity that is often an illustration of the delicate interplay of many components of the active immune system. In diagnosing the pathologic specimen, a basic approach is:1. To determine if the process is benign or malignant.2. If malignant, to characterise the origin of the malignancy.3. To subclassify or identify the process. In some cases, the first part—the question of benign versus malignant—cannot be resolved by histomorphologic evaluation alone. The second part of the approach has become considerably easier with the advent of immunophenotypic analysis. The final issue, that of identification of the pathologic process, may be of greater or lesser importance, depending on the clinical situation. If an exact diagnosis can be made it can have several benefits. In the case of neoplastic processes, it may indicate the type of treatment and the prognosis. In benign processes, there are other important benefits as well. Although some therapies may be instituted in benign conditions, often simply naming a thing can have great benefit to the patient. It is important to realise that even in the case of a purely reactive process, finding a name, a category, a classification, can provide real peace of mind to the patient. As the American Journal of Clinical Pathology declares, “The hundreds of images are the greatest asset of this book. There are extraordinary renditions of such exquisite classic (yet rarely illustrated) entities: ‘sago’ and ‘lardaceous’ spleens, ‘lollipop’ and ‘explosive’ lymphoid follicles, multinucleated measles cells and hyperplastic mesothelial inclusions, and sundry mystifying diseases, including those known eponymously: Kawasaki, Kikuchi, and Kimura.”For pathologists and hematopathologists specifically, the authors have created a book that will serve for years as a useful guide to the multifaceted world of benign and reactive lesions of the lymphoid system.

Since the publication of the third edition in 1997, the World Health Organization (WHO) has printed two major volumes concerning tumours of the thymus, and the authors of this volume collaborated on these books. In the last 10 years, little progress has been made in the study of tumours of the thymus, although a few new subtypes have been reported. The most significant change was the subtyping by the WHO of tumours of the thymus that do not cause constitutional changes. The present volume has adopted the WHO system of classification. For pathologists who are not as familiar with the WHO system, the authors clearly explain and illustrate both the morphologic and histogenetic classification systems that are its bases. Due to the changes in nomenclature, the chapter on thymic epithelial tumors has been considerably revised, although the main diagnostic terms are presented along with the previous conventional morphologic and histogenetic terms. Newly-recognised tumour entities, new cytologic and genetic findings, and recent references have been added, and the tables have been revised to reflect new data on survival, staging, and classification. This volume will be an authoritative reference on mediastinal tumours for years to come.

Placental diagnoses provide risk assessment for infant prognosis, recurrent risk for future pregnancies, and explanatory findings. There is widespread interest in placental diagnosis for the antenatal determinants of adult disease, inborn errors of metabolism, specific infections, malignancies, and assessing placental function in real time with the ultimate goal of affecting treatment. Due to the difficulties inherent in studying human gestation and gestational pathologies, much of what is known about the relevance of placental pathology is biased toward personal experience and retrospective studies. However, this book presents evidence-based information to the reader. The authors emphasize the need for an educated and directed sampling of the placenta through gross examination which will result in a better and more useful final diagnosis.

In surgical pathology practice, distinguishing malignant from benign melanocytic lesions represents one of the major problem areas. Failure to diagnose a malignant melanoma can have catastrophic results, but separating melanomas from their simulants is often difficult, and over-diagnosis of melanoma is also inappropriate. This edition presents a comprehensive account of benign and malignant melanocytic lesions. The benign lesions encompass the major simulants of melanoma. In addition, some of them, such as dysplastic nevi, have significance as markers of individuals at increased risk for the development of melanoma. It has become ever more apparent, especially with the recent advent of molecular classification, that melanoma is not one disease, and that prognosis in melanoma can vary considerably from lesions that present minimal risk, to dangerous malignancies at opposite ends of a spectrum. The details of therapy for these lesions are very different and are determined for the most part by their diagnostic and prognostic features as judged by microscopy. This edition explains in detail the use of attributes for microstaging and for prediction of prognosis in melanoma, including discussion of the latest AJCC staging system. More importantly, however, it provides a richly-illustrated roadmap for diagnosis of the entire spectrum of melanocytic tumours. Discussion of the lesions is firmly grounded in their clinical significance, with detailed descriptions of clinical features often accompanied by clinical images.Lesions in general are illustrated in multiple microscopic images ranging from scanning magnification to high power. Cytologic as well as architectural features are clearly delineated. Pathophysiology, epidemiology, and molecular attributes are also discussed. This work will serve as a comprehensive text for learning on the part of trainees, and also as an invaluable diagnostic reference for more experienced diagnosticians in daily practice.

This latest version of the Atlas of Non-melanocytic Tumors of the Skin is rich in illustrations, aimed to highlight important diagnostic aspects of individual cutaneous epithelial, mesenchymal, hematopoietic and lymphoid tumors, and to delineate their morphologic spectrum. The images are supplemented by concise text to emphasize the salient clinical and histologic features, differential diagnosis, and the use of immunohistochemistry and other ancillary tests. The goal is to provide a practical diagnostic guide that will serve as a useful resource for dermatopathology and surgical pathology practice.

In their classification, there were four variants of urothelial neoplasia, some adenomas, adenocarcinoma, and smooth muscle and lymphoid tumors.Today the classification continues to accrue entities based upon morphology, immunophenotype, and compelling clinical associations. The classification of renal cell carcinomas has undergone a profound transformation, with the emergence of many new entities, often with characteristic genetic alterations, with significant implications for prognosis and treatment. This comprehensive, detailed, and highly illustrated fascicle is a remarkable resource to surgical pathologists and other specialists who treat patients with tumors of these sites.

Since the 4th Series of this Fascicle was published, there have been considerable advances in our understanding of tumors within the serosal membranes, in particular the classification of mesothelial tumors and their genetics. This 5th Series Fascicle aligns with the terminology recommended by the WHO in its most recent edition for pleural and pericardial tumors, with expansion to reflect serosal membranes at other sites and including recognition of mesothelioma in situ.

The truth is simple—the opportunity to see many cases. The authors of this 5th Series Fascicle have such expertise and share a remarkable range of cases from their institutions as well as from many pathologists throughout the world who have sent them cases in consultation over the years. This fascicle is rich source of material that will be a great resource for practicing pathologists and pathologists in training.

The authors of this new fourth edition recognise that nomenclature for cysts and neoplasms of the jaws is dynamic, as demonstrated by the evolving schemes based on previously unrecognised microscopic, clinical, radiographic, and therapeutic features. This edition is not an attempt to promote one nomenclature scheme versus another, but simply to present the best information available for each lesion discussed using the most current World Health Organization classification system. While this edition continues in the American Registry of Pathology tradition of providing the most current diagnostic information available for the practising pathologist, in some cases surgeons will consult with pathologists regarding therapeutic management, hence information about peer-reviewed treatment protocols for many tumours and cysts is included in each section. This edition promises to be a quality resource for pathologists, surgeons, and dentists alike for many years.

This is unfortunate as nonneoplastic endocrine disorders are many times more common in the population. As in all fields of medicine, nonneo¬plastic endocrine diseases can closely mimic the neoplastic; an awareness of all aspects of pathology underpins routine clinical practice. This Fifth Series Fascicle offers pathologists and clinicians an opportunity to develop an increased understanding of nontumor endocrine pathology in order to inform patient care.

Although female lower genital tract pathology is largely dominated by epithelial tumors, the pathologist will also encounter mesenchymal, melanocytic, and hematopoietic neoplasms. This fascicle provides a comprehensive discussion of these categories with emphasis on their wid¬ening clinical and pathologic spectrum as well as their evolving molecular landscape. The book conveys the authors’ extensive experience as pathologists devoted to women’s health and to ad¬vancing understanding of gynecologic disease.

The current multimodal study of pancreatic neoplasms has allowed a better understanding of tumor biology, more closely linking the mor¬phologic findings with clinical outcomes and targeted treatment. The prognosis of conventional pancreatic ductal adenocarcinoma remains very poor, but some promise is on the horizon based on novel treatment approaches and the increase in earlier diagnosis. Furthermore, intensive study of other pancreatic neoplasms has led to a clearer recognition that each type has distinctive biology and behavior that is often not so highly aggressive. This sense of optimism may help drive a resurgence of interest in pancreatic pathology.

The complexity of ovary and fallopian tube pathology is striking, with dozens of tumor types. There is a greater appreciation now of the importance of hereditary cancer syndromes in ovarian and tubal malignancies, as two common autosomal dominant hereditary cancer predisposition syndromes include carcinomas of ovary or fallopian tube. This book is organized following the structure and terminology of the World Health Organization (WHO) classification of Female Genital Tumors, 5th edition. It includes some new entities that have been described since that book's publication in 2020 and provides detailed discussion of differential diagnosis, relevant ancillary tests, and strategies to approach diagnosis. In addition, discussions of the underlying biology of these tumors, as it relates to their diagnosis, are also presented. This fascicle provides an opportunity for deeper reading and quiet reflection about the progress that has been made in our understanding of tumors of the ovary since the previous landmark edition was published in 1998, as well as a respect for the numerous challenges that remain.Free digital access available with every print purchase. This new volume on the heart and great vessels has been expanded to become a comprehensive atlas of all cardiovascular disease, both neoplastic and non-neoplastic. In the spirit of prior editions, correlation of expertly curated pathology photographs and photomicrographs with clinical presentation, imaging and molecular genetics remains the cornerstone. The spectrum of cardiovascular disease, both congenital and acquired, is vast; accounting for the relative size of this fascicle. Every attempt has been made to focus on the clinically-relevant features of the diseases helping not only pathologists, but also providing cardiologists, radiologists, and surgeons a reference to understand the pathologic basis of the conditions they diagnose and treat each day. The authors have meticulously provided gross photographs that mirror (where possible) planar imaging techniques that will enable pathologic-radiologic correlation and facilitate a multidisciplinary approach to patient care.Free digital access available with every print purchase. The authors of this volume have put emphasis on diagnosis, so the presentation is generally concise, yet more comprehensive on key points. In addition, they emphasise diagnostically-important information on immunohistochemistry and molecular genetics, and include additional details when biologically significant. Site-specific soft tissue entities covered in the other atlases are often excluded due to space constraints. The text is aimed as a practical diagnostic aid for pathologists, cytopathologists and pathology trainees, but the authors hope that their clinical colleagues and others may also find it as a useful source of information on soft tissue tumours and tumour-like lesions.

In the years since the publication of the third edition, awareness of the remarkable morphologic diversity of thyroid carcinoma has notably expanded, and great advances have been made in unravelling the molecular genetic features of thyroid and parathyroid tumours. There has also been an increased interest in the use of the fine-needle aspiration technique for the diagnosis and management of thyroid tumours, and in the role of the pathologist in the operative handling of the hyperfunctioning parathyroid gland. This volume documents the most significant advances that have taken place since the previous edition, emphasising those with a practical application at the clinical level. The format remains similar to that of the previous versions, but the two thyroid and parathyroid editions have been merged into one, and most of the black and white images and photomicrographs have been replaced with colour. The authors include a number of the most recent references, but have not ignored the classic works in the field, many of which have descriptions, illustrations, and insights that cannot be bettered. This edition fulfils the original goal of this series, which is that of helping the pathologist diagnose and anticipate the behavior of the tumours and tumour-like lesions included in this publication.

The current edition contains new information accumulated during the last ten years on tumours and tumour-like lesions of the gallbladder, extrahepatic bile ducts and Vaterian system. Because of the increasing number of laparoscopic cholecystectomies and Whipple procedures performed, surgical pathologists now have the opportunity to examine more cancer precursors, early cancers, and adenomas of the gallbladder, extrahepatic bile ducts and ampulla of Vater than previously. These lesions are discussed in detail and profusely illustrated.Likewise, lesions that closely simulate malignant neoplasms are also described and illustrated. Also included are the giant cell neoplasms and the gastrointestinal stromal tumours as well as the new subtypes of carcinomas such as large cell neuroendocrine carcinomas and adenocarcinomas with pyloric gland or foveolar phenotype. The role of immunohistochemistry in the diagnosis of a wide variety of neoplasms is emphasised as well. This book will be a great resource to practicing pathologists and students for years to come.

Evolution in the field of diagnostic hematopathology has been remarkable. The World Health Organization (WHO) classification scheme for hematologic neoplasms has achieved widespread endorsement and acceptance in clinical practice. This volume integrates proposed WHO 2016 recommendations and highlights areas in which further classification revisions may occur, or areas in which criteria are controversial. In addition, the immunophenotypic and molecular genetic tests of greatest value in diagnoses, risk assessment, and disease monitoring are highlighted in the text. The substantial focus on molecular genetic findings for each neoplasm parallels the ever-expanding role of genetics in disease classification and risk assessment.Key problem areas and differential diagnostic considerations are also included for each neoplastic disease category. Although primary blood and bone marrow neoplasms are the focus of this text, metastatic neoplasms in bone marrow are also covered. This atlas will provide readers with comprehensive information necessary for the clinical assessment of bone marrow neoplasms for years to come.

Much has changed in the detection, diagnosis, and management of breast diseases since the previous editions of this fascicle, in particular for patients with invasive breast carcinoma. This fifth series edition conveys the challenges of diagnostic breast pathology, as well as excitement about the advances that have been, and have yet to be, made in the field.Access the content online for free with every print purchase.

The understanding of diagnostic lymph node and spleen pathology has evolved at an impressive pace since 1995, when the third series AFIP atlas was published. Progress has been fuelled, in large part, by the application of a number of technologies to the study of hematopoietic neoplasms. The World Health Organization (WHO) classification scheme for hematologic neoplasms has captured much of this progress, and is generally accepted for clinical practice. The current volume, part of the fourth series of atlases, integrates the WHO classification, including the 2016 revision, and highlights areas in which the WHO classification is likely to evolve with time. In keeping with the goals and tradition of the atlases, this volume emphasizes morphological features of hematopoietic neoplasms, and includes a discussion of differential diagnoses for each disease category. In addition, the results of immunophenotypic and molecular genetic testing for each disease are highlighted, including findings generated by using high-throughput techniques that likely will be integrated into standard diagnostics in the near future. Overall, this atlas will provide readers with the comprehensive information necessary for the pathological and clinical assessment of lymph node and splenic lesions.

There have been many advances in the pathology of intestinal tumours since the publication of the Third Series Intestines Fascicle in 2003, but many of the foundations of intestinal tumour diagnosis remain tried and true. Tubular adenomas are still tubular adenomas, but better understanding of serrated polyps has been a key advance in the years since the publication of the Third Series volume. Additionally, developments in molecular biology of colorectal carcinoma have allowed for targeted therapy and refinements to our evaluation of Lynch syndrome, which was termed hereditary nonpolyposis colorectal carcinoma (HNPCC) in the past. Our understanding of other polyposis syndromes has similarly blossomed in the past 15 years. Neuroendocrine tumours have been reclassified in the 2010 World Health Organization classification of gastrointestinal tumours. The molecular basis of gastrointestinal stromal tumours of the intestines has been a subject of great interest as well.In producing this update, this group of authors has enjoyed working together in gathering images and information to update this edition of the Intestines atlas. In doing so, we stand on the shoulders of giants before us, namely Drs. Robert H. Riddell, Robert E. Petras, Geraint T. Williams, and Leslie H. Sobin. While many areas of intestinal tumour pathology are without controversy, the classification and nomenclature for appendiceal tumours remain a subject of debate, including among ourselves. We have attempted to offer information on the various divergent viewpoints where they exist for this topic and others. Our efforts have been synergistic, and we hope that readers will enjoy the many interesting illustrations that we were able to amass.

Since the publication of the previous edition of Tumors of the Esophagus and Stomach, great advances in many areas of tumor biology have led to a better understanding of the pathogenesis, pathology, and molecular biology of epithelial and stromal malignancies of the upper gastrointestinal tract. Many of these advances have led to specific improvements in diagnosis, prognosis, treatment, and thus, survival in patients affected by these tumors. This edition of the Fascicle was written to highlight these advances, and more specifically, to help pathologists diagnose diseases more accurately and understand how pathology contributes to clinical treatment in the new age of personalized and targeted therapy.Notable advances described in this publication include:1) expansion of our understanding of the pathologic features and molecular pathogenesis of carcinomas of the esophagus and stomach, most of which develop through a chronic inflammation-metaplasia-dysplasia-carcinoma sequence. Important refinements in the classification of neoplastic precursor lesions have been made since the last AFIP series, including the morphologic and endoscopic subtypes, and this has led to improvements in surgical treatment with a strong trend toward minimization in the form of endoscopic mucosal and submucosal dissections2) discovery of new diseases, such as gastric adenocarcinoma and proximal polyposis syndrome (GAPPS), a special variant of familial adenomatous polyposis, and significant advances in our knowledge regarding the molecular characterization of genetic polyposis syndromes3) great expansion in the molecular-pathology correlation, morphologic diversity, classification, and therapeutic options of gastrointestinal stromal tumors (GISTs)4) reclassification and refinement of prognostic factors related to neuroendocrine tumors5) improvements in the role of cytology in the diagnosis of tumors of the upper gastrointestinal tract. This book has been written with the pathologist in mind first by using tables that illustrate salient pathologic, molecular, and differential diagnostic features of key entities that are often difficult to distinguish from each other. Specific color photographs of classic tumors and morphologic variants often difficult to recognize are used throughout.
