Denna bok om KOL är i stora delar en ny svensk lärobok och delvis en uppdaterad version av en bok med samma namn, utgiven av Studentlitteratur 2006. Utvecklingen inom KOL-området har varit oerhört framgångsrik under 2000-talet och många nya land­vinningar har gjorts. Kunskapen om de inflammatoriska mekanismer som styr sjukdomen har ökat och förståelsen för uppkomst och utveckling av sjukdomen har blivit större. KOL uppfattas mer och mer som en systemsjukdom, vilket innebär att behandlingsteamet alltmer kommit att närma sig diagnostik och behandling av sjukdomen ur ett tvärvetenskapligt perspektiv. Det nya synsättet har även medfört att specialister från olika medicinska discipliner engageras inom KOL-vården. Detta speglas i den aktuella boken i vilken förekomst, riskfaktorer, uppkomstmekanismer och sjukdomsförlopp noggrant beskrivs. Boken ger även utförlig information om de inflammatoriska processerna vid KOL och om de diagnostiska över­väganden som måste beaktas. KOL samvarierar med ett stort antal andra sjukdomstillstånd vilket belyses i flera kapitel. Vidare ges en bred översikt av de terapeutiska överväganden som man ska ta hänsyn till vid behandling av denna mångfacetterade sjukdom.Boken vänder sig i första hand till läkare inom den lungmedicinska specialiteten men även till läkare inom andra specialiteter, som intern- och allmänmedicin, vilka behandlar patienter med KOL. Denna andra upplaga av Lungmedicin rymmer stora sjukdomsgrupper som astma, kroniskt obstruktiv lungsjukdom och lungcancer, men även en rad viktiga, men mindre välkända tillstånd som sarkoidos och lungparenkymsjukdomar. I boken förmedlar ett betydande antal av landets främsta experter patogenes, kliniska symtom, förslag till utredningsgång och behandling vid olika sjukdomar i lungorna på ett lättillgängligt sätt. Utredning och omhändertagande av sjukdomar i luftvägar och lungor kräver ofta multidisciplinär samverkan och ingående kännedom om bland annat radiologi, patologi, fysiologi och onkologi för att uppnå goda resultat. Därför riktas boken till hela teamet som arbetar med lungpatienter. Kapitlen är genomgående uppdaterade eller nyskrivna.Syftet med boken är att öka kunskapsnivån hos blivande och färdiga lungspecialister, internmedicinare och specialister i allmänmedicin. Den kan med fördel även läsas av andra yrkeskategorier verksamma inom området, liksom av studerande på grundläggande hälso- och sjukvårdsutbildningar.

Det här är tredje upplagan av boken som nu bytt namn från Sjukgymnastik vid nedsatt lungfunktion till Fysioterapi vid nedsatt lungfunktion. Samtliga kapitel är uppdaterade och utvidgade. I övrigt föreligger ett antal mindre ändringar. Carl Lamm är överläkare med lång erfarenhet av thoraxradiologi. Han har arbetat i mer än 30 år på röntgenavdelningen vid Sahlgrenska Universitetssjukhuset i Göteborg. Hans uppskattade föreläsningar om HRCT i olika sammanhang har varit inspirationskälla till denna bok. Fredrik Ahlfors är specialistläkare med särskilt intresse för thoraxradiologi och HRCT. Han har arbetat som radiolog i 23 år, varav 10 år på Sahlgrenska Universitetssjukhuset i Göteborg. Fredrik Ahlfors är också verksam som skönlitterär författare.

Steg för steg beskrivs hur man analyserar undersökningarna, och i ett rikt bildmaterial pekas viktiga fynd ut som är typiska för normalfysiologi och andra som är typiska för olika sjukdomstillstånd. Tredje delen utgörs av ett 40-tal illustrativa fallbeskrivningar med utförliga analyser och förklaringar till hur olika fynd bör tolkas och vad de har för betydelse. Dels ges flera exempel på normala undersökningsfynd, dels ges ett stort antal exempel på typiska fynd vid olika sjukdomstillstånd. Stor vikt har lagts vid att identifiera de fynd i undersökningarna som bidrar till förståelse för grundfysiologin hos både friska och sjuka. Dessutom ges läsaren möjlighet att förstå hur man t ex skiljer hjärtsjukdom från lungsjukdom som orsak till en patients sänkta arbetsförmåga. Boken riktar sig till läkare, biomedicinska analytiker och fysioterapeuter som använder sig av ergo-​spirometri i sitt arbete, eller som vill få ökad förståelse för den här undersökningsmetoden. Generellt är förståelse för ergospirometri en väg till djupare förståelse för arbetsfysiologi hos både friska och sjuka.

The ICRP Publication 66 Human Respiratory Tract Model for Radiological Protection (HRTM) has been applied to calculate dose coefficients (doses per unit intake) and bioassay functions in ICRP Publications 68, 71, 72 and 78. For these purposes, ICRP assigned numerical values to a range of model parameters, such as the size of the inhaled particles and the breathing rate of the subjects. These are known as `default’ or `reference’ values, and were chosen to be typical, representative values. In any particular situation the actual values of many parameters can be considerably different from the reference values. Usually, doses from intakes of radionuclides are low compared with the relevant limit or constraint, and the resulting difference is unimportant. There are, however, circumstances where more reliable assessments of intake and dose are desirable. This Guidance Document therefore gives advice on applying specific information within the framework of the HRTM for assessing occupational and environmental exposures and for interpreting bioassay data. Chapters on each aspect of the model (morphometry, physiology, deposition, clearance, gases and vapours, dosimetry) provide: A summary of how the HRTM treats that topic; Information on the reference values of relevant parameters; Guidance on choosing between default values; Information on how doses and bioassay quantities (lung retention, urine, and faecal excretion) vary with the values of selected parameters, giving guidance on the importance of obtaining specific information; Simple examples of the use of specific information relating to the topic. Annexes give additional information for those directly involved in applying the HRTM to specific situations, including guidance on obtaining parameter values. Several detailed examples are also provided. One involves assessment of an individual’s intake and committed dose from comprehensive bioassay monitoring data.
