A strong chemical workforce in the United States will be essential to the ability to address many issues of societal concern in the future, including demand for renewable energy, more advanced materials, and more sophisticated pharmaceuticals. High school chemistry teachers have a critical role to play in engaging and supporting the chemical workforce of the future, but they must be sufficiently knowledgeable and skilled to produce the levels of scientific literacy that students need to succeed. To identify key leverage points for improving high school chemistry education, the National Academies' Chemical Sciences Roundtable held a public workshop, summarized in this volume, that brought together representatives from government, industry, academia, scientific societies, and foundations involved in outreach programs for high school chemistry teachers. Presentations at the workshop, which was held in August 2008, addressed the current status of high school chemistry education; provided examples of public and private outreach programs for high school chemistry teachers; and explored ways to evaluate the success of these outreach programs.

Biomarkers can be defined as indicators of any biologic state, and they are central to the future of medicine. As the cost of developing drugs has risen in recent years, reducing the number of new drugs approved for use, biomarker development may be a way to cut costs, enhance safety, and provide a more focused and rational pathway to drug development. On October 24, 2008, the IOM's Forum on Drug Discovery, Development, and Translation held "Assessing and Accelerating Development of Biomarkers for Drug Safety," a one-day workshop, summarized in this volume, on the value of biomarkers in helping to determine drug safety during development.

The financial crisis that began in 2008 is a stark demonstration that we as a nation take great risks when we build too much of our economy on a base that does not create real value. Relying on vaporous transactions to generate wealth is no substitute for making real products and providing real services. In the 21st century, the United States and the rest of the world will face some of the greatest challenges of the modern age: feeding a growing population, generating adequate energy without destroying the environment, countering chronic and emerging infectious diseases. The first decade of the new century has shown that technological innovation is essential for the United States and other countries to meet these challenges. At the 2009 Annual Meeting of the National Academy of Engineering in Irvine, California, a public forum entitled 'Rebuilding a Real Economy: Unleashing Engineering Innovation' brought together seven prominent leaders of the innovation system to discuss the challenges facing America. The insights of the panel members cut to the heart of what this nation needs to do to remain a global leader in the turbulent world of the 21st century.This summary captures the main points made by the forum participants with the aim of encouraging further reflection and discussion. As the panelists pointed out, no single action can reenergize our innovation system. A portfolio of interconnected and interdependent initiatives must be undertaken to generate new knowledge and technology and move that new knowledge successfully into a competitive world marketplace. But the panelists clarified the goal toward which we must strive and some of the most important steps we need to take to achieve that goal.

The rapid growth of home health care has raised many unsolved issues and will have consequences that are far too broad for any one group to analyze in their entirety. Yet a major influence on the safety, quality, and effectiveness of home health care will be the set of issues encompassed by the field of human factors research--the discipline of applying what is known about human capabilities and limitations to the design of products, processes, systems, and work environments. To address these challenges, the National Research Council began a multidisciplinary study to examine a diverse range of behavioral and human factors issues resulting from the increasing migration of medical devices, technologies, and care practices into the home. Its goal is to lay the groundwork for a thorough integration of human factors research with the design and implementation of home health care devices, technologies, and practices. On October 1 and 2, 2009, a group of human factors and other experts met to consider a diverse range of behavioral and human factors issues associated with the increasing migration of medical devices, technologies, and care practices into the home.This book is a summary of that workshop, representing the culmination of the first phase of the study.

Newborn screening samples are used to test more than 4 million infants each year for life-threatening diseases that are treatable if found at birth. These specimens also represent a potentially invaluable resource for public health and biomedical research. The IOM held a workshop to examine issues surrounding the use of residual specimens for translational research.

Tuberculosis (TB) kills approximately 4,500 people worldwide every day. While most cases of TB can be treated with antibiotics, some strains have developed drug resistance that makes their treatment more expensive, more toxic and less effective for the patient. The IOM Forum on Drug Discovery, Development, and Translation and the Academy of Science of South Africa held a workshop to discuss ways to fight the growing threat of drug-resistant TB.

Despite the many basic research discoveries in genetics, relatively few gene-based treatments, drugs, or preventative measures have been developed. One way to bridge this gap may be for industry, academia, and government to develop partnerships that share resources while distributing risk. However, intellectual property protections and other barriers can inhibit collaborative efforts. The Institute of Medicine held a workshop on July 22, 2010, to explore these issues and develop solutions.

An estimated 2 billion people, one third of the global population, are infected with Mycobacterium tuberculosis, the bacterium that causes tuberculosis. Spread through the air, this infectious disease killed 1.7 million in 2009, and is the leading killer of people with HIV. Tuberculosis (TB) is also a disease of poverty--the vast majority of tuberculosis deaths occur in the developing world. Exacerbating the devastation caused by TB is the growing threat of drug-resistant forms of the disease in many parts of the world. Drug-resistant tuberculosis presents a number of significant challenges in terms of controlling its spread, diagnosing patients quickly and accurately, and using drugs to treat patients effectively. In Russia in recent decades, the rise of these strains of TB, resistant to standard antibiotic treatment, has been exacerbated by the occurrence of social, political, and economic upheavals. The Institute of Medicine (IOM) Forum on Drug Discovery, Development, and Translation, in conjunction with the Russian Academy of Medical Sciences held a workshop to discuss ways to fight the growing threat of drug-resistant TB.The New Profile of Drug-Resistant Tuberculosis in Russia: A Global and Local Perspective: Summary of a Joint Workshop presents information from experts on the nature of this threat and how it can be addressed by exploring various treatment and diagnostic options.

Engineers know what they mean by the word technology. They mean the things engineers conceive, design, build, and deploy. But what does the word global in the phrase global technology mean? Does it mean finding a way to feed, clothe, house, and otherwise serve the 9 billion people who will soon live on the planet? Does it mean competing with companies around the world to build and sell products and services? On a more immediate and practical level, can the rise of global technology be expected to create or destroy U.S. jobs? The National Academy of Engineering held a three-hour forum exploring these and related questions. The forum brought together seven prominent members of the engineering community: * Esko Aho, Executive Vice President of Corporate Relations and Responsibility, Nokia; former Prime Minister of Finland * Bernard Amadei, Founder, Engineers Without Borders, Professor, University of Colorado * John Seely Brown, Visiting Professor, University of Southern California; Former Chief Scientist of Xerox Corporation * Ruth A. David, President and CEO of Analytic Services, Inc. * Eric C.Haseltine, Consultant, former Associate Director for Science and Technology in the Office of the Director of National Intelligence, and former head of research and development at Disney Imagineering * Nicholas Negroponte, Founder, One Laptop Per Child Association Inc., Founder and Chairman Emeritus of the MIT Media Lab * Raymond S. Stata, Co-founder and Chairman of the Board, Analog Devices Inc. In the first half of the forum, each panelist explored a specific dimension of the global spread of technology. The topics varied widely--from reducing poverty to the impact of young people on technology to the need for systems thinking in engineering. But all seven presenters foresaw a world in which engineering will be fundamentally different from what it has been. In the second half of the forum, the panelists discussed a variety of issues raised by moderator Charles Vest and by forum attendees.

Demographic changes, immigration, economic upheavals, and changing societal mores are creating new and altered structures, processes, and relationships in American families today. As families undergo rapid change, family science is at the brink of a new and exciting integration across methods, disciplines, and epistemological perspectives. The purpose of The Science of Research on Families: A Workshop, held in Washington, DC, on July 13-14, 2010, was to examine the broad array of methodologies used to understand the impact of families on children's health and development. It sought to explore individual disciplinary contributions and the ways in which different methodologies and disciplinary perspectives could be combined in the study of families. Toward an Integrated Science of Research on Families documents the information presented in the workshop presentations and discussions. The report explores the idea of family research as being both basic and applied, offering opportunities for learning as well as intervention. It discusses research as being most useful when organized around particular problems, such as obesity or injury prevention.Toward an Integrated Science of Research on Families offers a problem-oriented approach that can guide a broad-based research program that extends across funders, institutions, and scientific disciplines.

At some point during 2009, more than 17 million households in the United States had difficulty providing enough food for all their members because of a lack of resources. In more than one-third of these households, the food intake of some household members was reduced and normal eating patterns were disrupted due to limited resources. The Workshop on Understanding the Relationship Between Food Insecurity and Obesity was held to explore the biological, economic, psychosocial, and other factors that may influence the relationship between food insecurity, overweight, and obesity in the United States. Hunger and Obesity examines current concepts and research findings in the field. The report identifies information gaps, proposes alternative approaches to analyzing data, recommends new data that should be collected, and addresses the limitations of the available research.

From Neurons to Neighborhoods: An Update: Workshop Summary is based on the original study From Neurons to Neighborhoods: Early Childhood Development, which released in October of 2000. From the time of the original publication's release, much has occurred to cause a fundamental reexamination of the nation's response to the needs of young children and families, drawing upon a wealth of scientific knowledge that has emerged in recent decades. The study shaped policy agendas and intervention efforts at national, state, and local levels. It captured a gratifying level of attention in the United States and around the world and has helped to foster a highly dynamic and increasingly visible science of early childhood development. It contributed to a growing public understanding of the foundational importance of the early childhood years and has stimulated a global conversation about the unmet needs of millions of young children. Ten years later, the Board on Children, Youth, and Families of the Institute of Medicine (IOM) and the National Research Council (NRC) held a 2-day workshop in Washington, D.C., to review and commemorate a decade of advances related to the mission of the report.The workshop began with a series of highly interactive breakout sessions in which experts in early childhood development examined the four organizing themes of the original report and identified both measurable progress and remaining challenges. The second day of the workshop, speakers chosen for their diverse perspectives on early childhood research and policy issues discussed how to build on the accomplishments of the past decade and to launch the next era in early childhood science, policy, and practice. From Neurons to Neighborhoods: An Update: Workshop Summary emphasizes that there is a single, integrated science of early childhood development despite the extent to which it is carved up and divided among a diversity of professional disciplines, policy sectors, and service delivery systems. While much work still remains to be done to reach this goal, the 2010 workshop demonstrated both the promise of this integrated science and the rich diversity of contributions to that science.

Natural disasters are having an increasing effect on the lives of people in the United States and throughout the world. Every decade, property damage caused by natural disasters and hazards doubles or triples in the United States. More than half of the U.S. population lives within 50 miles of a coast, and all Americans are at risk from such hazards as fires, earthquakes, floods, and wind. The year 2010 saw 950 natural catastrophes around the world--the second highest annual total ever--with overall losses estimated at $130 billion. The increasing impact of natural disasters and hazards points to increasing importance of resilience, the ability to prepare and plan for, absorb, recover from, or more successfully adapt to actual or potential adverse events, at the individual , local, state, national, and global levels. Assessing National Resilience to Hazards and Disasters reviews the effects of Hurricane Katrina and other natural and human-induced disasters on the Gulf Coast of Louisiana and Mississippi and to learn more about the resilience of those areas to future disasters.Topics explored in the workshop range from insurance, building codes, and critical infrastructure to private-sector issues, public health, nongovernmental organizations and governance. This workshop summary provides a rich foundation of information to help increase the nation's resilience through actionable recommendations and guidance on the best approaches to reduce adverse impacts from hazards and disasters.

The enactment of the America COMPETES Act in 2006 (and its reauthorization in 2010), the increase in research expenditures under the 2009 American Recovery and Reinvestment Act (ARRA), and President Obama's general emphasis on the contribution of science and technology to economic growth have all heightened interest in the role of scientific and engineering research in creating jobs, generating innovative technologies, spawning new industries, improving health, and producing other economic and societal benefits. Along with this interest has come a renewed emphasis on a question that has been asked for decades: Can the impacts and practical benefits of research to society be measured either quantitatively or qualitatively? On April 18-19, 2011, the Board on Science, Technology, and Economic Policy (STEP) and the Committee on Science, Engineering and Public Policy (COSEPUP) of the National Academy of Sciences, the National Academy of Engineering, and the Institute of Medicine, held a workshop to examine this question. The workshop sought to assemble the range of work that has been done in measuring research outcomes and to provide a forum to discuss its method.The workshop was motivated by a 2009 letter from Congressman Rush Holt (D-New Jersey). He asked the National Academies to look into a variety of complex and interconnected issues, such as the short-term and long-term economic and non-economic impact of federal research funding, factors that determine whether federally funded research discoveries result in economic benefits, and quantification of the impacts of research on national security, the environment, health, education, public welfare, and decision making. Measuring the Impacts of Federal Investments in Research provides the key observations and suggestions made by the speakers at the workshop and during the discussions that followed the formal presentations.

An estimated 8.8 million people fell ill with tuberculosis (TB) in 2010 and 1.4 million died from the disease. Although antibiotics to treat TB were developed in the 1950s and are effective against a majority of TB cases, resistance to these antibiotics has emerged over the years, resulting in the growing spread of multidrug-resistant (MDR) TB. Due to challenges in timely and accurate diagnosis of drug-resistant TB, length and tolerability of treatment regimens, and expense of second-line anti-TB drugs, effectively controlling the disease requires complex public health interventions. The IOM Forum on Drug Discovery, Development, and Translation held three international workshops to gather information from local experts around the world on the threat of drug resistant TB and how the challenges it presents can be met. Workshops were held in South Africa and Russia in 2010. The third workshop was held April 18-19, 2011, in New Delhi, India, in collaboration with the Indian National Science Academy and the Indian Council of Medical Research.The aim of the workshop was to highlight key challenges to controlling the spread of drug-resistant strains of TB in India and to discuss strategies for advancing and integrating local and international efforts to prevent and treat drug-resistant TB. This document summarizes the workshop.

The initial sequencing of the human genome, carried out by an international group of experts, took 13 years and $2.7 billion to complete. In the decade since that achievement, sequencing technology has evolved at such a rapid pace that today a consumer can have his or her entire genome sequenced by a single company in a matter of days for less than $10,000, though the addition of interpretation may extend this timeframe. Given the rapid technological advances, the potential effect on the lives of patients, and the increasing use of genomic information in clinical care, it is important to address how genomics data can be integrated into the clinical setting. Genetic tests are already used to assess the risk of breast and ovarian cancers, to diagnose recessive diseases such as cystic fibrosis, to determine drug dosages based on individual patient metabolism, and to identify therapeutic options for treating lung and breast tumors, melanoma, and leukemia.With these issues in mind and considering the potential impact that genomics information can have on the prevention, diagnosis, and treatment of disease, the Roundtable on Translating Genomic-Based Research for Health hosted a workshop on July 19, 2011, to highlight and identify the challenges and opportunities in integrating large-scale genomic information into clinical practice. Integrating Large-Scale Genomic Information into Clinical Practice summarizes the speaker presentations and the discussions that followed them. This report focuses on several key topics, including the analysis, interpretation, and delivery of genomic information plus workforce, ethical, and legal issues.

The demand for health care is growing as the nation ages and seeks to provide coverage for the millions of Americans who lack health insurance. At the same time, escalating costs have led to a variety of initiatives to make the delivery of health care more effective and efficient. The allied health workforce is critical to the success of these efforts. The IOM held a workshop May 9-10, 2011, to examine the current allied health care workforce and consider how it can contribute to improving health care access, quality, and effectiveness.

The development and application of regulatory science - which FDA has defined as the science of developing new tools, standards, and approaches to assess the safety, efficacy, quality, and performance of FDA-regulated products - calls for a well-trained, scientifically engaged, and motivated workforce. FDA faces challenges in retaining regulatory scientists and providing them with opportunities for professional development. In the private sector, advancement of innovative regulatory science in drug development has not always been clearly defined, well coordinated, or connected to the needs of the agency. As a follow-up to a 2010 workshop, the IOM held a workshop on September 20-21, 2011, to provide a format for establishing a specific agenda to implement the vision and principles relating to a regulatory science workforce and disciplinary infrastructure as discussed in the 2010 workshop.

After the September 11, 2001, terrorist attacks and subsequent anthrax mailings, the U.S. government prioritized a biosurveillance strategy aimed at detecting, monitoring, and characterizing national security health threats in human and animal populations, food, water, agriculture, and the environment. However, gaps and challenges in biosurveillance efforts and integration of biosurveillance activities remain. September 8-9, 2011, the IOM held a workshop to explore the information-sharing and collaboration processes needed for the nation's integrated biosurveillance strategy.

More than two decades ago, a commission of 17 MIT scientists and economists released a report, Made in America, which opened with the memorable phrase, "To live well, a nation must produce well." Is that still true? Or can the United States remain a preeminent nation while other countries increasingly make the products that once were made in America? These questions were at the center of a forum titled "Making Things: 21st Century Manufacturing and Design" held during the 2011 Annual Meeting of the National Academy of Engineering. In a wide-ranging and provocative conversation, seven leaders of business, government, and academia explored the many facets of manufacturing and design and outlined the many opportunities and responsibilities posed by manufacturing for the engineering profession. Making Things: 21st Century Manufacturing and Design summarizes the discussions that took place during the 2011 forum. The report concludes with a forum agenda and each attendee's bibliography.

The sequencing of the human genome and the identification of associations between specific genetic variants and diseases have led to an explosion of genomic-based diagnostic tests. These tests have the potential to direct therapeutic interventions, predict risk or onset of disease, or detect residual disease. As research progresses and an increasing number of associations are found, further tests will be developed that can aid in providing personalized treatment options for patients. However, the adoption of genomic diagnostic tests by health care providers has been limited due to a lack of evidence regarding the clinical utility of many tests. Health funders and practitioners lack the data necessary to distinguish which tests can improve practice or the clinical settings in which tests will provide the greatest value. The Roundtable on Translating Genomic-Based Research for Health held a workshop in November 2010 to determine what evidence is needed and how it is viewed by different stakeholders in order to develop genomic diagnostic tests of clinical value. Genome-Based Diagnostics summarizes the presentations and discussions that took place throughout the workshop.Two presentations, in particular, sparked extensive discussion. One presentation proposed that all genomic diagnostic tests be reviewed and approved by the Food and Drug Administration. The other observed that venture capitalists are no longer investing substantially in the development of genomic diagnostic tests because of a lack of clarity surrounding regulatory and reimbursement pathways. Both presentations suggested the need for major changes in the systems used to develop, regulate, and reimburse genomic diagnostic tests. The report also presents the perspectives of different stakeholders in the development of genomic diagnostic tests. Each stakeholder group has a different set of needs and issues of importance, yet commonalities among them are apparent, such as the need to put patients and health outcomes at the center of discussion and action.

The Chemical Sciences Roundtable (CSR) was established in 1997 by the National Research Council (NRC). It provides a science oriented apolitical forum for leaders in the chemical sciences to discuss chemistry-related issues affecting government, industry, and universities. Organized by the National Research Council's Board on Chemical Sciences and Technology, the CSR aims to strengthen the chemical sciences by fostering communication among the people and organizations - spanning industry, government, universities, and professional associations - involved with the chemical enterprise. One way it does this is by organizing workshops that address issues in chemical science and technology that require national attention. In September 2011, the CSR organized a workshop on the topic, "The Role of Chemical Sciences in Finding Alternatives to Critical Resources." The one-and-a-half-day workshop addressed key topics, including the economic and political matrix, the history of societal responses to key mineral and material shortages, the applications for and properties of existing minerals and materials, and the chemistry of possible replacements.The workshop featured several presentations highlighting the importance of critical nonfuel mineral and material resources in history, catalysis, agriculture, and electronic, magnetic, and optical applications. The Role of the Chemical Sciences in Finding Alternatives to Critical Resources: A Workshop Summary explains the presentations and discussions that took place at the workshop. In accordance with the policies of the NRC, the workshop did not attempt to establish any conclusions or recommendations about needs and future directions, focusing instead on issues identified by the speakers.

In October 2005, the National Academy of Sciences, National Academy of Engineering, and Institute of Medicine released a policy report that served as a call to action. The report, Rising Above the Gathering Storm: Energizing and Employing America for a Brighter Economic Future observed that "the scientific and technological building blocks critical to the United States economic leadership are eroding at a time when many other nations are gathering strength." The report laid out 20 recommendations in four broad areas - K-12 education, science and engineering research, higher education, and economic and technology policy - and warned that a failure to take action could have dire economic consequences. Rising Above the Gathering Storm sparked intense discussion among policy makers, industrial leaders, and the general public. Five years after the release of the Gathering Storm report, a second report, Rising Above the Gathering Storm, Revisited: Rapidly Approaching Category 5, assessed changes in America's competitive posture. This report concluded that "our nation's outlook has not improved, but rather has worsened" since the Gathering Storm report was released.The report noted examples of other nations that have upgraded their investments in education, technological infrastructure, and innovation systems to a greater extent than has the United States. The ability of the states to drive innovation was the impetus behind a major workshop held in Madison, Wisconsin, on September 20-22, 2011. Titled "Rising Above the Gathering Storm: Developing Regional Innovation Environments," the workshop brought together leaders in education, government, economic development, and industrial innovation to discuss state and regional initiatives to boost competitiveness through science, technology, and innovation. The conference was organized around four major themes: - Revitalizing K-12 Science and Mathematics Education - Strengthening Undergraduate Education in Science and Engineering - Building Effective Partnerships Among Governments, Universities, Companies, and Other Stakeholders - Fostering Regional Technology Development and Entrepreneurship Rising Above the Gathering Storm: Developing Regional Innovation Environments: A Workshop Summary gives an overview of the presentations, observations, and recommendations made during the workshop.

The National Research Council (NRC) and National Academy of Engineering (NAE) have released a new report, Community Colleges in the Evolving STEM Education Landscape: Summary of a Summit. Based on a national summit that was supported by the National Science Foundation and organized by the NRC and the NAE, the report highlights the importance of community colleges, especially in emerging areas of STEM (Sciene, Technology, Engineering, and Mathematics) and preparation of the STEM workforce. Community colleges are also essential in accommodating growing numbers of students and in retraining displaced workers in skills needed in the new economy. Community Colleges in the Evolving STEM Education Landscape: Summary of a Summit looks at the changing and evolving relationships between community colleges and four-year institutions, with a focus on partnerships and articulation processes that can facilitate student success in STEM; expanding participation of students from historically underrepresented populations in undergraduate STEM education; and how subjects, such as mathematics, can serve as gateways or barriers to college completion.

Societies have sought to improve the outputs of their agricultural producers for thousands of years. In the 19th and early 20th centuries, efforts to convey agricultural knowledge to farmers became known as extension services, a term adopted from programs at Oxford and Cambridge designed to extend the knowledge generated at universities to surrounding communities. Traditionally, extension services have emphasized a top-down model of technology transfer that encourages and teaches producers to use crop and livestock varieties and agricultural practices that will increase food production. More recently, extension services have moved toward a facilitation model, in which extension agents work with producers to identify their needs and the best sources of expertise to help meet those needs. On May 1, 2012, the Roundtable on Science, Technology, and Peacebuilding held a workshop in Washington, DC, to explore whether and how extension activities could serve peacebuilding purposes. The Roundtable is a partnership between the National Academy of Engineering (NAE) and the U.S. Institute of Peace (USIP).It consists of senior executives and experts from leading governmental organizations, universities, corporations, and nongovernmental organizations, was established in 2011 to make a measurable and positive impact on conflict management, peacebuilding, and security capabilities. Its principal goals are: * To accelerate the application of science and technology to the process of peacebuilding and stabilization; * To promote systematic, high-level communication between peacebuilding and technical organizations on the problems faced and the technical capabilities required for successful peacebuilding; and * To collaborate in applying new science and technology to the most pressing challenges for local and international peacebuilders working in conflict zones.
